摘 要:目的:探讨经鼻高流量湿化氧疗联合布地奈德吸入气雾剂对急性呼吸衰竭患者的影响。方法:回顾性分析2021年7月—2023年7月新余钢铁集团有限公司中心医院收治的72例急性呼吸衰竭患者的临床资料。按照治疗方法的不同进行分组,其中予以布地奈德吸入气雾剂治疗的36例患者为对照组,予以布地奈德吸入气雾剂+经鼻高流量湿化氧疗的36例患者为观察组。对比两组生命体征、氧合状况、呼吸动力学指标、急性生理与慢性健康评估Ⅱ(acute physiology and chronic health evaluationⅡ,APACHEⅡ)评分、不良反应。结果:治疗后,观察组心率、舒张压、收缩压、气道阻力、平均气道压、动脉血二氧化碳分压(arterial partial pressure of carbon dioxide,PaCO_(2))、APACHEⅡ评分均低于对照组,动脉血氧分压(arterial partial pressure of oxygen,PaO_(2))、血氧饱和度(oxygen saturation of blood,SaO_(2))、氧合指数(oxygenation index,OI)均高于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率相当,差异无统计学意义(P>0.05)。结论:在急性呼吸衰竭患者中应用经鼻高流量湿化氧疗联合布地奈德吸入气雾剂治疗能够改善患者生命体征,调节机体呼吸功能与氧合状态,且安全性高。Objective:To investigate the effect of nasal high-flow humidification oxygen therapy combined with Budesonide Inhalation Aerosol on patients with acute respiratory failure.Method:The clinical data of 72 patients with acute respiratory failure admitted to Xinyu Iron and Steel Group Co.,LTD.Central Hospital from July 2021 to July 2023 were retrospectively analyzed.The patients were divided into different groups according to different treatment methods.36 patients treated with Budesonide Inhalation Aerosol were used as control group,and 36 patients treated with Budesonide Inhalation Aerosol plus nasal high-flow humidification oxygen therapy were used as observation group.Vital signs,oxygenation,respiratory dynamics,acute physiology and chronic health evaluationⅡ(APACHEⅡ)score and adverse reactions were compared between the two groups.Result:After treatment,the heart rate,diastolic blood pressure,systolic blood pressure,airway resistance,mean airway pressure,arterial partial pressure of carbon dioxide(PaCO_(2))and APACHEⅡscore in the observation group were lower than those in the control group,arterial partial pressure of oxygen(PaO_(2)),oxygen saturation of blood(SaO_(2)),oxygenation index(OI)were higher than those of control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The application of nasal high-flow humidification oxygen therapy combined with Budesonide Inhalation Aerosol in the treatment of acute respiratory failure can improve the vital signs of patients,regulate the respiratory function and oxygenation status of the body,and have high safety.
