养心定悸胶囊对冠心病心律失常患者心率变异性及心率震荡的影响

摘　　要：目的:探究养心定悸胶囊对冠心病心律失常患者心率变异性及心率震荡的影响。方法:将2021年10月—2023年12月北京市昌平区中西医结合医院收治的80例冠心病心律失常患者根据随机数字表法分为两组,每组40例。对照组进行常规治疗,观察组在对照组的基础上加用养心定悸胶囊。比较两组治疗总有效率、治疗前后心律失常指标(房性期前收缩、室性期前收缩及房室交界性期前收缩)、心率变异性指标[相邻正常心搏间期之差>50 ms占总窦性心搏的百分比(PNN50)、相邻窦性心搏RR间期差值均方根(rMSSD)及窦性心搏RR间期标准差(SDNN)]及心率震荡指标[震荡初始(TO)、震荡斜率(TS)]。结果:观察组的治疗总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗前两组心律失常指标、心率变异性指标及心率震荡指标比较,差异均无统计学意义(P>0.05);治疗2、4周后,两组PNN50均高于治疗前,观察组均显著高于对照组,rMSSD、SDNN均长于治疗前,且观察组均显著长于对照组(P<0.05);治疗2、4周后,两组心律失常指标发生次数均显著少于治疗前,且观察组均显著少于对照组,差异均有统计学意义(P<0.05);治疗2、4周后,两组TO均显著小于治疗前,观察组均显著小于对照组,两组TS均显著大于治疗前,且观察组均显著大于对照组(P<0.05)。结论:养心定悸胶囊在冠心病心律失常患者中的应用效果好,可显著改善心率变异性及心率震荡。Objective:To investigate the effects of Yangxin Dingji Capsules on heart rate variability and heart rate turbulence in patients with coronary heart disease complicated with arrhythmia.Method:A total of 80 patients with coronary heart disease complicated with arrhythmia admitted to Changping District Hospital of Traditional Chinese and Western Medicine from October 2021 to December 2023 were divided into two groups according to random number table method,with 40 cases in each group.The control group received routine treatment,and the observation group received Yangxin Dingji Capsules on the basis of the control group.The total effective rate,arrhythmia indexes(premature atrial contraction,ventricular premature beat and junctional premature contraction),heart rate variability indexes[percentage of normal-to-normal intervals with differences greater than 50 milliseconds(PNN50),the root mean square of successive differences in RR intervals(rMSSD),the standard deviation of normal to normal RR intervals(SDNN)]and heart rate turbulence indexes[turbulence onset(TO)and turbulence slope(TS)]before and after treatment were compared between the two groups.Result:The total effective rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).There were no significant differences in arrhythmia indexes,heart rate variability indexes and heart rate turbulence indexes between the two groups before treatment(P>0.05).After 2 and 4 weeks of treatment,PNN50 in both groups were higher than those before treatment,and those in observation group were significantly higher than those in control group,rMSSD and SDNN were significantly longer than those before treatment,and those in observation group were significantly longer than those in control group(P<0.05).After 2 and 4 weeks of treatment,the frequency of arrhythmia indexes in both groups were significantly less than those before treatment,and those in the observation group were significantly less than those
