摘  要:目的研制一种带隔绝套筒和止位套筒的防护型呼吸回路,评估其在保护患者和医护人员中的应用价值。方法选取2020年8月至2021年8月于医院ICU行气管切开术且持续机械通气患者32例,按气道护理操作方式分为开放式吸痰(OES)组和密闭式吸痰(CES)组,所有患者均接受上述两种操作。比较操作前后两组心率、呼吸、平均动脉压(MAP)、血氧饱和度(SaO_(2))水平及操作结束后医护人员手部、周围环境中的细菌平均阳性率。结果操作前,两组生命体征指标比较,差异无统计学意义(P>0.05);操作后,CES组心率、呼吸、MAP均低于OES组,SaO2水平高于OES组(P<0.05)。操作结束后,CES组医护人员手部细菌平均阳性率、周围环境细菌平均阳性率均低于OES组(P<0.05)。结论防护型呼吸回路装置可维持患者生命体征稳定,减少医护人员交叉感染情况的发生。Objective To develop a protective breathing system with isolating sleeve and stop sleeve,and evaluate its application value in protecting patients and medical staff.Methods With the selection of 32 patients undergoing tracheotomy and continuous mechanical ventilation in the ICU of hospital from August 2020 to August 2021,they were divided into open endotracheal suction(OES)and closed endotracheal suction(CES)according to the operation method,and all patients underwent the above procedures.The changes of heart rate,respiration,mean arterial pressure(MAP)and blood oxygen saturation(SaO_(2))before and after the operation and the average positive rate of bacteria in the hands of medical staff and the surrounding environment after the operation were compared between the two groups.Results Before the operation,there were no statistically significant differences in vital sign indicators between the two groups(P>0.05);After the operation,the heart rate,respiration,and MAP in the CES group were lower than those in the OES group,and the SaO2 was higher than that in the OES group(P<0.05).After the operation,the average positive rate of bacteria in the hand and the surrounding environment in the CES group were lower than those in the OES group(P<0.05).Conclusion The protective breathing system device can maintain the stability of the patient's vital signs and reduce the occurrence of cross infection among medical staff.
