摘　　要：目的:分析连续性血液净化、血浆置换辅助治疗噬血细胞性淋巴组织细胞增生症(HLH)的效果。方法:选取贵阳市妇幼保健院2023年1—12月收治的50例HLH患儿为研究对象,采用随机数字表法分为两组。对照组实施基础治疗,观察组在对照组基础上实施连续性血液净化、血浆置换治疗。比较两组临床疗效、白细胞介素-6(interleukin-6,IL-6)、白细胞介素-10(interleukin-10,IL-10)、白细胞介素-12(interleukin-12,IL-12)、不良反应发生情况。结果:观察组总有效率为88.00%,高于对照组的60.00%,差异有统计学意义(χ^(2)=5.094,P=0.024);观察组治疗4周后IL-6、IL-10、IL-12水平均低于对照组,差异均有统计学意义(P<0.05);观察组不良反应总发生率为16.00%,低于对照组的48.00%,差异有统计学意义(χ^(2)=5.882,P=0.015)。结论:连续性血液净化、血浆置换辅助治疗HLH患儿的效果良好,能够显著降低炎症因子水平,减少不良反应发生率。Objective:To analyze the effect of continuous blood purification and plasmapheresis in the adjuvant treatment of hemophagocytic lymphohistiocytosis(HLH).Method:A total of 50 children with HLH treated in Guiyang Maternal and Child Health Care Hospital from January to December 2023 were selected as the study objects and divided into two groups according to random number table method.The control group received basic treatment,and the observation group received continuous blood purification and plasmapheresis on the basis of the control group.The clinical efficacy,interleukin-6(IL-6),interleukin-10(IL-10),interleukin-12(IL-12)and the incidence of complications were compared between the two groups.Result:The total effective rate of the observation group was 88.00%,which was higher than 60.00%of the control group,the difference was statistically significant(χ^(2)=5.094,P=0.024).After 4 weeks of treatment,the levels of IL-6,IL-10 and IL-12 in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 16.00%,which was lower than 48.00%in the control group,the difference was statistically significant(χ^(2)=5.882,P=0.015).Conclusion:Continuous blood purification and plasmapheresis have good effect in the adjuvant treatment of HLH children,which can significantly reduce the levels of inflammatory factors and reduce the incidences of complications.
