火针联合沙利度胺对结节性痒疹患者皮损情况及生活质量的影响

摘　　要：目的:探讨火针联合沙利度胺对结节性痒疹(PN)患者皮损情况及生活质量的影响。方法:选取2023年1月—2024年5月丰城市第三人民医院皮肤科收治的80例PN患者,根据随机数字表法分为对照组与治疗组,各40例。对照组口服沙利度胺片,治疗组在对照组的基础上联合火针进行治疗。对比两组治疗前后皮损情况、血清生化因子[肿瘤坏死因子(TNF)-α、P物质(SP)、白细胞介素(IL)-6]、生活质量[皮肤病生活质量指数(DLQI)],精神心理状态[贝克焦虑量表(BAI)、贝克抑郁量表(BDI)、匹兹堡睡眠质量指数(PSQI)、感知压力量表(PSS)评分],以及临床疗效、不良反应发生情况。结果:治疗前,两组皮损情况、血清生化因子水平、生活质量及精神心理状态比较,差异均无统计学意义(P>0.05);治疗后,与对照组比较,治疗组皮损情况评分、DLQI、BAI、BDI、PSQI、PSS评分及TNF-α、SP、IL-6水平均显著低,临床总有效率显著高,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:火针联合沙利度胺治疗PN的效果突出,对患者皮损情况、血清生化因子水平、生活质量及精神心理状态均有明显改善,且安全性较好。Objective:To investigate the effect of fire needle combined with Thalidomide on skin lesions and quality of life of patients with prurigo nodularis(PN).Method:A total of 80 PN patients admitted to the Department of Dermatology,Fengcheng Third People's Hospital from January 2023 to May 2024 were selected and divided into control group and treatment group according to random number table method,with 40 cases in each group.The control group took Thalidomide Tablets orally,and the treatment group was treated with fire needle on the basis of the control group.The skin lesions,serum biochemical factors[tumor necrosis factor(TNF)-α,substance P(SP)and interleukin(IL)-6],quality of life[dermatology life quality index(DLQI)]and mental and psychological state[Beck anxiety inventory(BAI),Beck depression inventory(BDI),Pittsburgh sleep quality index(PSQI)and perceived stress scale(PSS)scores]before and after treatment,and clinical efficacy and the incidence of adverse reactions of the two groups were compared.Result:There were no significant differences in skin lesions,serum biochemical factor levels,quality of life and mental and psychological state between the two groups before treatment(P>0.05).After treatment,compared with control group,skin lesions scores,the scores of DLQI,BAI,BDI,PSQI,PSS,and the levels of TNF-α,SP and IL-6 in the treatment group were significantly lower,the clinical total effective rate was significantly higher,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The effect of fire needle combined with Thalidomide on PN is outstanding,and the skin lesions,serum biochemical factors,quality of life and mental and psychological state of patients are significantly improved,and the safety is good.
