基于扶正祛邪理论齐元富教授治疗肿瘤的用药规律及临床经验总结

摘　　要：目的:探析基于扶正祛邪理论齐元富教授治疗肿瘤的用药规律,总结其临床诊疗经验。方法:选取2018年1月—2024年6月在山东中医药大学附属医院经齐元富教授治疗的肿瘤患者106例,收集患者病例首次处方并对处方药物进行归纳整理。使用频数统计学、聚类分析等数据挖掘技术总结基于扶正祛邪理论齐元富教授治疗肿瘤的用药规律及临床经验。结果:106例患者中,性别方面以男性居多,年龄段集中在41~50岁;疾病类型根据占比从高到低依次为肺癌、骨肉瘤、膀胱癌、前列腺癌。106首处方中包含154种中药,累积使用频次为1182次。处方使用频次高于30次的中药有23味,其中前三名依次为柴胡、黄柏、白术。106首处方治疗肿瘤药物根据频次从高到低进行排序,药性依次为温性药、凉性药、平性药;药味依次为苦味、甘味、辛味、涩味。药物归经涉及十二经,以肺经占比最高,其余依次为脾经、膀胱经、肝经、肾经、胃经、大肠经。组方规律分析提取使用频次在20次以上的核心药物共10个数据,其中2味药核心组合有9条,3味药核心组合有1条。通过无监督熵层次聚类算法得到治疗肿瘤新处方7首。结论:齐元富教授认为肿瘤的发生与正气不足、邪毒入侵密切相关,临证过程以扶正不留邪,祛邪不伤正为首要,灵活应用增强机体正气及袪除病邪之品。Objective:To explore the drug use of Professor Qi Yuanfu in the treatment of tumor based on Fuzheng Quxie theory,and to summarize his clinical experience in diagnosis and treatment.Method:A total of 106 tumor patients treated by Professor Qi Yuanfu in Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2018 to June 2024 were selected,and the first prescription of the patients was collected and summarized.Data mining techniques such as frequency statistics and cluster analysis were used to summarize the drug use rule and clinical experience of Professor Qi Yuanfu in the treatment of tumor based on Fuzheng Quxie theory.Result:Among the 106 patients,most of them were male and their age range was from 41 to 50 years old.The types of diseases were lung cancer,osteosarcoma,bladder cancer,and prostate cancer in descending order.106 prescriptions included 154 Chinese medicines,and the cumulative frequency was 1182 times.There were 23 Chinese medicines with prescription frequency higher than 30,and the top 3 frequencies were Bupleurum,Yellow cypress and Atractylodes.The 106 prescription tumor drugs were ranked from high to low according to the frequency,and the drug properties were warm drugs,cool drugs,and flat drugs in order;the medicinal taste was bitter,sweet,hot and astringent in order;the drug meridian involves twelve meridians,with the highest lung meridian,and the rest are spleen meridian,bladder meridian,liver meridian,kidney meridian,stomach meridian and large intestine meridian.A total of 10 data of core drugs used more than 20 times,including 9 core combinations of 2 drugs and 1 core combination of 3 drugs.7 new prescriptions for therapeutic tumors were obtained through the unsupervised entropy hierarchical clustering algorithm.Conclusion:Professor Qi Yuanfu believes that the occurrence of tumors is closely related to the lack of healthy Qi and the invasion of evil poison.In the process of clinical syndrome,it is the first thing to help the right and not leave the evil,remov
