AFHR治疗老年股骨粗隆间骨折患者的临床效果

摘　　要：目的:探讨人工股骨头置换术(AFHR)在老年股骨粗隆间骨折(IFF)患者中的应用价值。方法:选取60例老年IFF患者,均为兴仁市人民医院2021年1月—2023年6月收治,按随机数字表法分为两组,各30例。对照组行近端防旋髓内钉(PFNA)内固定术,观察组行AFHR。比较两组手术情况、髋关节功能、日常生活能力、下肢运动功能、生活质量及并发症。结果:与对照组比较,观察组手术时间长,下床活动时间早,住院时间短,术中出血量及术后引流量均多(P<0.05)。与对照组比较,术后6个月,观察组Harris评分、Barthel指数(BI)、下肢Fugl-Meyer评估(FMA)及生活质量评分均高,差异均有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:AFHR治疗老年IFF患者,可加快患者髋关节功能恢复,更好地改善日常生活能力及下肢活动能力,但术中出血量大,需临床依据病情特点慎重选择。Objective:To investigate the application value of artificial femoral head replacement(AFHR)in elderly patients with intertrochanteric fracture of femur(IFF).Method:A total of 60 elderly patients with IFF,all admitted to Xingren City People's Hospital from January 2021 to June 2023,and were divided into two groups according to random number table method,with 30 cases in each group.The control group underwent proximal femoral nail antirotation(PFNA)internal fixation,and the observation group underwent AFHR.Surgical status,hip function,daily living ability,lower limb motor function,quality of life and complications were compared between the two groups.Result:Compared with the control group,the operation time in the observation group was longer,getting out of bed was earlier and hospitalization was shorter,intraoperative blood loss and postoperative drainage volume were more(P<0.05).Compared with the control group,6 months after surgery,Harris score,Barthel index(BI),Fugl-Meyer assessment(FMA)of lower limb and quality of life scores in observation group were higher,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:The treatment of AFHR on elderly patients with IFF can accelerate the recovery of hip function,better improve the ability of daily living and lower limb mobility,but the intraoperative blood loss is large,need to be carefully selected according to the clinical characteristics of the disease.
