全膝关节置换术后患者膝关节线改变程度及其与膝前痛的相关性分析

摘　　要：目的:分析全膝关节置换术后患者膝关节线改变程度及其与膝前痛的相关性。方法:回顾性分析2023年1—8月上饶市人民医院收治的80例行全膝关节置换术患者。测量术前、术后12个月患者的膝关节线,并计算膝关节线变化程度。评估术前、术后12个月患者Feller评分、美国膝关节协会评分(KSS),并分析全膝关节置换术后患者膝前痛与膝关节线改变程度的相关性。结果:术前全膝关节置换术患者Feller评分低于术后12个月[(10.83±2.46)分vs(18.70±3.13)分](P<0.05);术前全膝关节置换术患者膝关节功能评分低于术后12个月[(45.57±4.13)分vs(74.46±3.91)分](P<0.05);术前全膝关节置换术患者膝评分低于术后12个月[(57.37±4.52)分vs(87.29±3.48)分](P<0.05)。全膝关节置换术后膝关节水平改变-5.0~10.0 mm,平均2.50(-1.25,6.25)mm,膝关节线下降24例,升高56例,其中膝关节线改变水平≤4 mm有30例,膝关节线改变水平>4 mm有50例。术后12个月,膝关节线改变>4 mm患者Feller评分、膝关节功能评分、膝评分均低于≤4 mm患者,差异均有统计学意义(P<0.05)。当膝关节线改变≤4 mm,膝关节线改变程度与Feller评分无相关性(rs=-0.441,P=0.274),当膝关节线改变>4 mm,膝关节线改变程度与Feller评分呈负相关性(rs=-0.745,P=0.000)。结论:全膝关节置换术后可改变膝关节线,当膝关节线改变>4mm时,可明显影响患者的膝关节功能,引起膝前痛。Objective:To analyze the degree of knee line change in patients after total knee arthroplasty and its correlation with anterior knee pain.Method:A total of 80 patients underwent total knee arthroplasty admitted to Shangrao People's Hospital from January to August 2023 were analyzed retrospectively.The knee line of patients was measured before surgery and 12 months after surgery,and the degree of knee line change was calculated.Feller score and American knee society score(KSS)were evaluated before surgery and 12 months after surgery,and the correlation between anterior knee pain and the degree of knee line change after total knee arthroplasty was analyzed.Result:The Feller score of patients with total knee arthroplasty before surgery was lower than that of 12 months after surgery[(10.83±2.46)scores vs(18.70±3.13)scores](P<0.05).The knee joint function score of patients with total knee arthroplasty before surgery was lower than that of 12 months after surgery[(45.57±4.13)scores vs(74.46±3.91)scores](P<0.05).The knee score of patients with total knee arthroplasty before surgery was lower than that of 12 months after surgery[(57.37±4.52)scores vs(87.29±3.48)scores](P<0.05).After total knee arthroplasty,the knee line level changed-5.0-10.0 mm,with an average of 2.50(-1.25,6.25)mm,the knee line decreased in 24 cases and increased in 56 cases,including 30 cases with the knee line change≤4 mm and 50 cases with the knee line change>4 mm.12 months after surgery,Feller score,knee joint function score and knee score in patients with knee line change>4 mm were lower than those in patients with≤4 mm,the differences were statistically significant(P<0.05).When the knee line change≤4 mm,the knee line change degree was not correlated with Fellers score(rs=-0.441,P=0.274),when the knee line change>4 mm,the knee line change degree was negatively correlated with Fellers score(rs=-0.745,P=0.000).Conclusion:After total knee arthroplasty,the knee line can be changed.When the knee line change>4 mm,it can significantly affect
