血清人绒毛膜促性腺激素、AMH、孕酮联合检测对先兆流产孕妇的预后价值分析

摘　　要：目的:分析血清人绒毛膜促性腺激素、抗米勒管激素(AMH)、孕酮联合检测对先兆流产孕妇预后的评估价值。方法:选取宜春市妇幼保健院2021年1月—2023年10月收治的101例先兆流产孕妇作为研究对象进行回顾性分析,并根据妊娠结局分为P组(妊娠失败,n=42)和C组(继续妊娠,n=59)。对比两组血清人绒毛膜促性腺激素、AMH、孕酮水平,分析三者单独及联合检测对先兆流产孕妇妊娠失败的预测价值。分析先兆流产孕妇妊娠结局的影响因素。结果:P组与C组年龄、孕周、孕前体重指数、孕妇类型、产次、受孕方式、孕前吸烟史、孕前饮酒史、孕前1年接受过X线检查情况、甲亢情况比较,差异均无统计学意义(P>0.05);P组孕前患有妇科疾病、文化程度为高中以下占比均显著高于C组,人绒毛膜促性腺激素、AMH、孕酮水平均显著低于C组(P<0.05)。多因素logistic回归分析显示:孕前患有妇科疾病、文化程度为高中以下均为先兆流产孕妇妊娠失败的独立危险因素(P<0.05);人绒毛膜促性腺激素、AMH、孕酮水平均为先兆流产孕妇妊娠失败的独立保护因素(P<0.05)。血清人绒毛膜促性腺激素、AMH、孕酮单项及三者联合检测的曲线下面积(AUC)均>0.7,三者联合预测先兆流产孕妇妊娠失败的特异度、敏感度分别为92.86%、94.92%。结论:孕前患有妇科疾病、文化程度为高中以下均为先兆流产孕妇妊娠失败的独立危险因素,人绒毛膜促性腺激素、AMH、孕酮水平均为先兆流产孕妇妊娠失败的独立保护因素,人绒毛膜促性腺激素、AMH、孕酮水平联合预测先兆流产孕妇妊娠失败的价值高。Objective:To analyze the evaluate value of serum human chorionic gonadotrophin,anti-Müllerian hormone(AMH)and progesterone combined detection in gravida with threatened abortion.Method:A total of 101 gravida with threatened abortion admitted to Yichun Maternal and Child Health Hospital from January 2021 to October 2023 were analyzed retrospectively,and were divided into group P(pregnancy failure,n=42)and group C(continue the pregnancy,n=59)according to the pregnancy outcomes.The levels of serum human chorionic gonadotrophin,AMH and progesterone were compared between the two groups,and the predictive value of single and combined detection of the three in gravida with threatened abortion were analyzed.The influencing factors of pregnancy outcome in gravida with threatened abortion was analyzed.Result:There were no significant differences in age,gestational week,body mass index before pregnancy,gravida type,delivery time,conception method,smoking history before pregnancy,drinking history before pregnancy,X-ray examination 1 year before pregnancy and hyperthyroidism between group P and group C(P>0.05).The proportions of with gynecological diseases before pregnancy and education level below high school in group P were significantly higher than those in group C,and the levels of human chorionic gonadotropin,AMH and progesterone were significantly lower than those in group C(P<0.05).Multivariate logistic regression analysis showed that with gynecological diseases before pregnancy and education level below high school were independent risk factors for pregnancy failure in gravida with threatened abortion(P<0.05).Human chorionic gonadotropin,AMH and progesterone levels were independent protective factors for pregnancy failure in gravida with threatened abortion(P<0.05).The area under the curve(AUC)of serum human chorionic gonadotropin,AMH and progesterone were all>0.7,and the specificity and sensitivity of the combined detection of the three were 92.86%and 94.92%,respectively.Conclusion:With gynecological diseases before
