摘　　要：目的:探讨腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)对急性结石性胆囊炎(acute calculous cholecystitis,AAC)患者胃肠功能、炎症因子的影响。方法:将2023年5月—2024年5月上海市普陀区中心医院收治的180例ACC患者纳入本研究,按照随机数表法分为观察组与对照组,各90例。对照组接受开腹手术治疗,观察组接受LC治疗。比较两组术后胃肠功能恢复时间、并发症发生情况及手术前后白细胞介素-8(interleukin-8,IL-8)、C反应蛋白(C-reactive protein,CRP)水平。结果:观察组术后正常进食时间、肛门排气时间、肠鸣音恢复时间短于对照组,并发症总发生率低于对照组,差异有统计学意义(P<0.05);术后,两组CRP和IL-8水平较术前升高,但观察组低于对照组,差异有统计学意义(P<0.05)。结论:与开腹手术相比,LC能促进ACC患者胃肠功能恢复,炎症反应更轻,并发症更少。Objective:To explore the effect of laparoscopic cholecystectomy(LC)on gastrointestinal function and inflammatory factors in patients with acute calculous cholecystitis(AAC).Method:A total of 180 patients with ACC who admitted to Putuo District Central Hospital,Shanghai from May 2023 to May 2024 were included in this study,they were divided into the observation group and the control group according to the random number table method,90 cases in each group.The control group was given laparotomy,and the observation group was given LC treatment.The recovery time of gastrointestinal function,the incidence of complications and the levels of interleukin-8(IL-8)and C-reactive protein(CRP)before and after operation were compared between two groups.Result:The postoperative normal eating time,exhaust time and recovery time of bowel sounds in the observation group were shorter than those in the control group,and the total incidence of complications was lower than that in the control group,the differences were statistically significant(P<0.05);after operation,the levels of CRP and IL-8 in two groups were higher than those before operation,but those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).Conclusion:Compared with laparotomy,LC can promote the recovery of gastrointestinal function in patients with ACC,with lighter inflammatory response and fewer complications.
