摘　　要：目的:分析输尿管软镜联合负压吸引治疗感染性肾结石的临床效果。方法:选取2023年1月—2024年1月毕节市中医医院收治的154例感染性肾结石患者。按随机数表法分为两组,各77例。对照组采用输尿管软镜碎石术;观察组在应用输尿管软镜进行碎石时联合负压吸引治疗。对比两组结石清除率、肾功能等。结果:观察组术后1周、4周结石清除率分别为75.32%、96.10%,高于对照组的59.74%、81.82%,手术操作时间、肾造瘘管留置时间短于对照组,术中肾盂内压低于对照组,差异有统计学意义(P<0.05)。观察组术后2 h各项感染性指标含量均较对照组低,差异有统计学意义(P<0.05)。两组术后3 d肾功能指标比较,差异无统计学意义(P>0.05)。观察组术后并发症发生率较对照组低,差异有统计学意义(P<0.05)。结论:输尿管软镜联合负压吸引治疗感染性肾结石取得较好效果,结石清除率高,手术操作时间短,并发症少。Objective:To analyze the clinical effect of ureteral soft scope combined with negative pressure aspiration in the treatment of infectious kidney stones.Method:A total of 154 patients with infectious kidney stones were selected from Bijie Hospital of Traditional Chinese Medicine from January 2023 to January 2024.They were divided into two groups by random number table method,with 77 cases in each group.Ureteral soft scope was used for lithotripsis in control group.The observation group was treated with negative pressure aspiration when ureteral soft scope was used for lithotripsis.The calculus clearance rate and renal function were compared between the two groups.Result:The stone clearance rates of the observation group were 75.32%and 96.10%at 1 week and 4 weeks after surgery,respectively,which were higher than 59.74%and 81.82%of the control group,the differences were statistically significant(P<0.05).The operative time and indwelling time of the nephrostomy tube were shorter than those of the control group,and the intraoperative pressure in the renal pelvis was lower than that of the control group,the differences were statistically significant(P<0.05).The infectivity index content of the observation group at 2 h after operation was lower than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in renal function between the two groups at 3 d after surgery(P>0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The combination of ureteral soft scope and negative pressure suction therapy has achieved good results in the treatment of infectious kidney stones,with high stone clearance rate,short surgical time,and few complications.
