集束化护理联合中医手指点穴预防老年髋部骨折下肢深静脉血栓的效果

摘　　要：目的:观察集束化护理联合中医手指点穴预防老年髋部骨折下肢深静脉血栓(deep vein thrombosis,DVT)的效果。方法:选取2022年1月—2023年12月南京中医药大学附属盐城中医院收治的80例老年髋部骨折患者作为研究对象。利用随机表法将上述患者分为观察组(n=40)和对照组(n=40),对照组患者实施常规护理,观察组患者实施集束化护理联合中医手指点穴。比较两组中医症状评分、凝血指标、Caprini血栓风险评估量表评分、Brathel指数评定量表评分和Harris髋关节评分量表评分及DVT发生率。结果:观察组Caprini量表评分和中医症状评分均低于对照组,差异有统计学意义(P<0.05)。出院前1 d,两组凝血指标与入院当日相比,差异有统计学意义(P<0.05);对照组出院前1 d,凝血酶原时间、凝血酶时间长于观察组,纤维蛋白原高于观察组,差异有统计学意义(P<0.05)。入院当日,两组Harris量表评分对比,差异无统计学意义(P>0.05);出院前1 d,两组Harris量表评分均高于入院当日,相较于对照组,观察组患者Harris量表评分显著提升,差异有统计学意义(P<0.05)。入院当日,两组患者Brathel指数评分对比,差异无统计学意义(P>0.05);出院前1 d,两组Brathel指数评分均高于入院当日,对照组Brathel指数评分低于观察组,差异有统计学意义(P<0.05)。观察组DVT发生率为2.50%(1/40),低于对照组的17.50%(7/40),差异有统计学意义(P<0.05)。结论:集束化护理联合中医手指点穴可显著改善老年髋部骨折患者中医症状评分,降低DVT发生率,提升日常生活能力和髋关节功能,以缩短治疗周期,降低患者及其家庭负担。Objective:To observe the effect of cluster nursing combined with traditional Chinese medicine finger point for preventing lower limb deep vein thrombosis(DVT)in the elderly patients with hip fracture.Method:A total of 80 elderly patients with hip fracture admitted to Yancheng Hospital of Traditional Chinese Medicine Affiliated to Nanjing University of Chinese Medicine from January 2022 to December 2023 were selected as the study objects.Randomized table method was used to divide the above patients into observation group(n=40)and control group(n=40).Patients in the control group received routine nursing,and patients in the observation group received cluster nursing combined with traditional Chinese medicine finger point.Traditional Chinese medicine symptom score,blood clotting index,Caprini thrombosis risk assessment scale score,Brathel index rating scale score,Harris hip joint score score and the incidence of DVT were compared between the two groups.Result:The Caprini scale score and traditional Chinese medicine symptom score of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).1 d before discharge,the blood coagulation indexes of the two groups had statistical significance compared with the day of admission(P<0.05);1 d before discharge,prothrombin time and thrombin time in control group were longer than those in observation group,and fibrinogen was higher than those in observation group,the difference was statistically significant(P<0.05).On the day of admission,there was no significant difference in Harris scale scores between the two groups(P>0.05).1 d before discharge,the Harris scale scores of both groups were higher than those on the day of admission,and compared with the control group,the Harris scale scores of patients in the observation group were significantly improved,the difference was statistically significant(P<0.05).On the day of admission,there was no significant difference in Brathel index score between the two groups(P>0.0
