摘　　要：目的:对比新固-曼组合环[固定式节育器(新固)与左炔诺孕酮宫内缓释系统(LNG-IUS,商品名曼月乐)组合]与LNG-IUS在环移位、脱落患者中的效果。方法:选取2021年1月—2023年5月在昆山市第五人民医院诊治的发生环下移或脱落的84例子宫腺肌症、异常子宫出血(AUB)、痛经、子宫内膜增生、子宫内膜息肉患者作为研究对象,按照随机数表法分为观察组(放置新固-曼组合环)和对照组(放置LNG-IUS),各42例。随访18个月,比较两组环下移、脱环情况,比较两组治疗前、治疗18个月后痛经程度[口述分级评分法(VRS)]、月经量[月经失血图(PBAC)]、血红蛋白(Hb)水平及子宫体积。结果:观察组随访18个月环移位和脱落率低于对照组,差异有统计学意义(P<0.05);治疗18个月后,两组Hb水平高于治疗前,VRS评分、PBAC少于治疗前,子宫体积小于治疗前,且观察组优于对照组,差异有统计学意义(P<0.05)。结论:新固-曼组合环治疗环移位、脱落患者环再移位、脱落率低,且治疗后VRS评分、PBAC、Hb水平及子宫体积优于LNG-IUS。Objective:To compare the effects of New Gu-Man combined ring[a combination of a fixed-type contraceptive device(New Gu)and Levonorgestrel Intrauterine Sustained-release System(LNG-IUS,trade name Mirena)]and LNG-IUS in patients with ring displacement,expulsion.Method:A total of 84 patients with adenomyosis,abnormal uterine bleeding(AUB),dysmenorrhea,endometrial hyperplasia,and endometrial polyps who had ring displacement or expulsion and treated in the Fifth People's Hospital of Kunshan City from January 2021 to May 2023 were selected as the research objects,they were divided into the observation group(placed with New Gu-Man combined ring)and the control group(placed with LNG-IUS)according to the random number table method,with 42 patients in each group.The patients were followed up for 18 months,the ring displacement,expulsion were compared between two groups,the degree of dysmenorrhea[oral grading score(VRS)],menstrual volume[pictorial blood loss assessment chart(PBAC)],hemoglobin(Hb)level and uterine volume were compared between two groups before and after 18 months of treatment.Result:The rate of ring displacement and expulsion in the observation group was significantly lower than that in the control group at 18 months follow up,and the difference was statistically significant(P<0.05);after 18 months of treatment,the Hb levels of two groups were higher than those before treatment,VRS scores PBAC were less than those before treatment,uterine volume were less than those before treatment,and those in the observation group were better than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The rate of ring re-displacement,expulsion of patients with ring displacement,expulsion treated with New Gu-Man combined ring is low,and the VRS score,PBAC,Hb level and uterine volume after treatment are better than those of LNG-IUS.
