扶阳保肾方对糖尿病肾病合并Ⅳ型心肾综合征患者NT-proBNP及心脏超声EF值的影响

摘　　要：目的:探讨扶阳保肾方在糖尿病肾病合并Ⅳ型心肾综合征(CRS)患者中的应用疗效。方法:选取我院2022年11月—2023年6月收治的心肾阳虚型糖尿病肾病合并Ⅳ型CRS患者120例,按照随机数字表法分为两组,对照组(60例)给予常规治疗,研究组(60例)给予常规治疗联合扶阳保肾方,对比两组临床疗效、中医证候积分、心脏功能指标[左心室舒张末期内径(LVDD)、左房内径(LAD)、左室射血分数(LVEF)]、实验室指标[收缩压、舒张压、氨基末端脑钠肽前体(NT-proBNP)、估计的肾小球滤过率、血肌酐(Scr)、血尿素氮(BUN)]、不良反应。结果:两组总有效率比较差异有统计学意义(P<0.05);治疗后两组中医证候积分评分均降低(P<0.05),且研究组中医证候积分均低于对照组(P<0.05);治疗后两组LVDD、LAD均降低,LVEF均升高(P<0.05),且研究组LVDD、LAD均低于对照组,LVEF高于对照组(P<0.05);治疗后两组收缩压、舒张压、NT-proBNP、Scr、BUN水平均降低,GFR水平升高(P<0.05),且研究组NT-proBNP、Scr、BUN水平均低于对照组,GFR水平高于对照组(P<0.05);治疗期间血尿常规、肝肾功能及不良反应发生率相当(P>0.05)。结论:扶阳保肾方可改善心肾阳虚型糖尿病肾病合并Ⅳ型CRS患者的疗效,改善心功能及相关实验室指标,且不良反应未见明显增加,安全性良好。Objective:To investigate the therapeutic effect of Fuyang Baoshen prescription in patients with diabetic nephropathy complicated with typeⅣcardio-renal syndrome(CRS).Methods:A total of 120 patients with heart-kidney-yang deficiency type diabetic nephropathy complicated with typeⅣCRS admitted to our hospital from November 2022 to June 2023 were divided into two groups according to random number table method.The control group(60 cases)received conventional treatment,and the study group(60 cases)received conventional treatment combined with Fuyang Baoshen prescription.Clinical efficacy,TCM symptom score,cardiac function indexesleft ventricular end-diastolic diameter(LVDD),left atrial diameter(LAD),left ventricular ejection fraction(LVEF),laboratory indexes Systolic blood pressure,diastolic blood pressure,amino terminal brain natriuretic peptide precursor(NT-proBNP),glomerular filtration rate(GFR),blood creatinine(Scr),blood urea nitrogen(BUN)were compared between the two groups.,adverse reactions.Results:Two group total effective rate was significantly difference(P<0.05);Two groups after treatment of TCM syndrome integral scores were lower(P<0.05),and the research of TCM syndrome integral are lower than the control group(P<0.05);In the two groups after treatment LVDD,LAD reduced,LVEF were higher(P<0.05),and the team LVDD,LAD were lower than the control group,LVEF was higher than the control group(P<0.05);Two groups after treatment systolic pressure,diastolic pressure,NT-proBNP,Scr,the level of BUN was lower,GFR levels(P<0.05),and the team NT-proBNP,Scr and BUN levels are lower than the control group,the GFR level higher than the control group(P<0.05);The incidence of hematuria routine,liver and kidney function and ADR were similar during treatment(P>0.05).Conclusion:Fuyang and kidney-protecting can improve the efficacy of patients with heart-kidney-yang deficiency diabetic nephropathy complicated with typeⅣCRS,improve cardiac function and related laboratory indexes,and no significant increase in adverse reacti
