摘 要:目的观察通气散对气滞血瘀型暴聋mTOR信号活性的调控作用,探寻治疗感音神经性听力损失的新靶点。方法纳入气滞血瘀型暴聋患者30例,健康志愿者30例,应用Western Blot法检测两组外周血单核细胞PBMC中mTOR、4EBP1和P70S6K蛋白表达水平;暴聋组在指南常规治疗基础上加服通气散浓缩煎剂,10日观察周期结束,纯音听阈评价疗效,复测mTOR、4EBP1和P70S6K蛋白表达。结果①健康对照组与暴聋组外周血PBMC检测,mTOR、4EBP1和P70S6K蛋白表达有统计学差异(P<0.001)。②暴聋治疗有效18例,无效12例;仅有效组疗后mTOR、4EBP1和P70S6K蛋白表达水平较疗前有统计学差异(P<0.001)。③有效组与无效组在性别、年龄、病程基线水平无统计学差异,但两组耳聋听力分型具有显著差异(P=0.001)。结论通气散可通过激活mTOR信号活性并磷酸化下游靶蛋白参与气滞血瘀型暴聋的治疗,但预期可能受暴聋不同发病机制的影响。Objective To observe the regulatory effect of Tongqi powder on mTOR signaling activity in patients with sudden deafness of Qi stagnation and blood stasis,and explore additional targets for the treatment of sensorineural hearing loss.Methods The subjects of study were 30 cases with sudden deafness of Qi stagnation and blood stasis,and 30 healthy volunteers.Western blot was performed to detect the protein expressions of mTOR,4EBP1 and P70S6K in peripheral blood mononuclear cells(PBMCs)of the two groups of patients.The sudden deafness group received concentrated decoction of Tongqi powder in addition to routine treatment according to the treatment guidelines,and the treatment ended after 10 days of observation period.The therapeutic efficacy was evaluated based on pure tone audiometry,and protein expressions of mTOR,4EBP1 and P70S6K were measured repeatedly.Results①There were statistical differences in the protein expressions of mTOR,4EBP1 and P70S6K in PBMCs between the healthy control group and the sudden deafness group(P<0.0001).②In the sudden deafness group,18 cases were effectively treated,and 12 cases were ineffectively treated;and only the effective group showed statistically significant differences in the comparison of pre-and post-treatment mTOR,4EBP1 and P70S6K protein expressions(P<0.0001).③There was no statistically significant difference between the effective group and the ineffective group in gender,age,and disease course at baseline,but significant difference was found in hearing classification of deafness between the two groups(P=0.001).Conclusion Tongqi powder can treat sudden deafness of Qi stagnation and blood stasis by activating mTOR signaling and phosphorylating downstream target proteins,but the effect may be affected by different pathogenesis of sudden deafness.
