穴位按摩对鼻内镜术后留置硅胶扩张引流管患者术后早期舒适状况的影响

摘　　要：目的探讨结合头部穴位按摩护理方案对功能性鼻内镜术后留置硅胶扩张引流管患者术后早期生活质量的影响。方法选取2022年2月~2023年12月期间,于北京同仁医院耳鼻咽喉头颈外科行功能性鼻内镜手术后,48小时内持续留置硅胶引流通气管的112例患者为研究对象,按入院先后顺序分为对照组和观察组,每组各56例。对照组采取一般常规护理,观察组在此基础上联合实施头部穴位按摩法,术后6小时、12小时、24小时分别实施头部穴位按摩,按摩结束后30min,采用疼痛数字评分法、视觉模拟(VAS)评分法分别进行各时间点的术后早期护理效果评估。结果按上述方案分别实施头部穴位按摩后30分钟,与干预前及对照组比较,患者局部疼痛程度均较干预前明显减轻,差异均有统计学意义(Z值分别为-3.425、-4.543、-2.712,均P<0.05),患者总体不适状况均较干预前明显减轻,差异均有统计学意义(Z值分别为-4.681、-5.305、-3.396,均P<0.05)。结论在常规护理基础上,结合头部穴位按摩早期干预,能有效改善鼻内镜术后留置硅胶扩张引流管患者的疼痛程度,提升术后早期的舒适水平,值得临床推广应用。Objective To observe the effects of head acupoint massage combined with routine nursing on the comfort level and life quality during the earlier postoperative period of patients with silicone dilating drainage tube in the sinus ostium following nasal endoscopic surgery.Methods Included in this trial were 112 cases,collected from Feb.2022 to Dec.2023 at our department,undergone nasal endoscopic surgery with silicone dilating drainage tube placed in the sinus ostium following the operation and retained there for 48 hrs.They were divided into 2 group according to the admission order,i.e.observation group(OG)and control one(CG),with 56 cases in each group.The cases in CG were given routine nursing procedures,while those in OG were nursed by that used in CG combined with head acupoint massage,carried out at these time points of 6,12 and 24 hrs after the operation respectively,performed 30 min.each time.Then,the scores of pain digital scale and visual analog scale(VAS)were used to evaluate the nursing effects during the earlier postoperative period on these patients by the end of each acupoint massage.Results After each head acupoint massage lasted for 30 min.,the pain level of patients in OG was significantly reduced,with statistical significance(Z values:-3.425、-4.543、-2.712,P<0.05)and the overall discomfort feeling of patients was also obviously improved,when compared with that before intervention and those cases in CG,also with statistical significance(Z values:-4.681、-5.305、-3.396,P<0.05).Conclusion It has been shown that this kind of nursing program combined routine procedures with head acupressure can effectively improve the pain status and comfort level during the earlier postoperative period of patients with silicone dilated drainage tube in the sinus ostium following nasal endoscopic surgery and is worthy of further clinical promotion.
