ABO同型异体红细胞悬液输注对Rh血型系统C、E抗原双群频率的影响及输血疗效分析

摘　　要：目的:分析ABO同型异体红细胞悬液输注对Rh血型系统C、E抗原双群频率的影响及输血疗效。方法:选取本院2023年1月至12月收治的120例输血治疗患者,随机分为观察组与对照组,各60例;观察组输注ABO及Rh-D血型相同而RhC、RhE表型不同的红细胞悬液2U,对照组输注ABO及Rh-D血型相同且RhC、RhE表型相同的红细胞悬液2U;观察两组患者输血后Rh血型C、E抗原双群频率、输血疗效、免疫功能变化及不良反应发生情况。结果:观察组在输注异体红细胞悬液后Rh血型C、E抗原双群频率均高于对照组(P<0.05);观察组患者输血后的红细胞计数(RBC)、血红蛋白(Hb)及血细胞比容(HCT)水平均低于对照组(P<0.05);观察组输血后1、7 d的外周血CD3+及CD4+水平均低于对照组(P<0.05);观察组不良反应发生率高于对照组(P<0.05)。结论:ABO同型异体红细胞悬液输注治疗中,RhC、E不同型输血会增加抗原双群频率,影响输血疗效,增强机体免疫抑制反应,增加输血不良反应的发生率。Objective:To analyze the impact of ABO homologous allogeneic red blood cell suspension transfusion on the double-group frequency of Rh blood group system C and E antigens and the efficacy of blood transfusion.Methods:A total of 120 patients who underwent blood transfusion treatment in our hospital from January to December 2023 were selected and randomly divided into an observation group and a control group,with 60 patients in each.The observation group received 2U of red blood cell suspension with the same ABO and Rh-D blood types but different RhC and RhE phenotypes,while the control group received 2U of red blood cell suspension with the same ABO and Rh-D blood types and the same RhC and RhE phenotypes.The double-group frequency of Rh blood group C and E antigens,the efficacy of blood transfusion,changes in immune function,and the occurrence of adverse reactions were observed in both groups after transfusion.Results:The double-group frequency of Rh blood group C and E antigens in the observation group was higher than that in the control group after allogeneic red blood cell suspension transfusion(P<0.05).The levels of red blood cell count(RBC),hemoglobin(Hb),and hematocrit(HCT)in the observation group were lower than those in the control group after blood transfusion(P<0.05).The levels of peripheral blood CD3+and CD4+in the observation group were lower than those in the control group on days 1 and 7 after blood transfusion(P<0.05).The incidence of adverse reactions in the observation group was higher than that in the control group(P<0.05).Conclusion:In ABO homologous allogeneic red blood cell suspension transfusion therapy,blood transfusion with different RhC and E phenotypes increases the double-group frequency of antigens,affects the efficacy of blood transfusion,enhances the immune suppressive response of the body,and increases the incidence of adverse reactions related to blood transfusion.
