血清CRP、IL-1β、IL-6表达水平与冠心病心肌缺血后心电图波形的关系

摘　　要：目的:分析血清C反应蛋白(CRP)、白介素-1β(IL-1β)、白细胞介素-6(IL-6)表达水平与冠心病心肌缺血后心电图波形的关系。方法:将收治的66例冠心病患者临床资料按是否发生心肌缺血进行分组,其中33例冠心病心肌缺血患者作为观察组,33例冠心病非心肌缺血患者作为对照组;两组患者均进行心电图检查和血清CRP、IL-1β、IL-6表达水平的检测;比较两组患者血清CRP、IL-1β、IL-6表达水平、心电图波形;分析血清CRP、IL-1β、IL-6表达水平与冠心病心肌缺血后心电图波形的相关性;明确CRP、IL-1β、IL-6及心电图波形异常指标对冠心病心肌缺血的预测价值。结果:观察组CRP、IL-1β、IL-6水平均高于对照组,差异显著(P<0.01);两组RR间期、T波时限、PR间期比较差异不显著(P>0.05),观察组QRS时限低于对照组,差异显著(t=2.064,P=0.043);心电图波形变化指标QRS时限与CRP、IL-1β、IL-6呈显著负相关关系(r=-0.541,P=0.005、r=-0.614,P=0.000、r=-0.592,P=0.001);血清CRP、IL-1β、IL-6表达水平及QRS时限联合对冠心病心肌缺血有良好的预测价值,受试者操作特征曲线下面积(AUC)最大,为0.960,灵敏度、特异度较高,分别为0.925、0.968;CRP、IL-1β、IL-6、QRS时限的预测阈值分别为65.385 mg/L、22.060 ng/L、127.620 pg/ml、110.840 ms,AUC分别为0.898、0.890、0.918、0.601。结论:血清CRP、IL-1β、IL-6表达水平与冠心病心肌缺血后心电图波形QRS时限呈负相关,CRP、IL-1β、IL-6及QRS时限对冠心病心肌缺血有良好的预测价值。Objective:The study examined the correlation between serum C-reactive protein(CRP),interleukin-1β(IL-1β),and interleukin-6(IL-6)levels and ECG waveforms during myocardial ischemia in coronary heart disease patients.Methods:Clinical data of 66 patients with coronary heart disease admitted to our hospital from March 2020 to October 2023 were collected and divided into groups according to whether myocardial ischemia occurred.33 patients with coronary heart disease and myocardial ischemia were taken as the observation group,and 33 patients with coronary heart disease and non-myocardial ischemia were taken as the control group.Ecg and serum CRP,IL-1βand IL-6 expression levels were detected in both groups.The expression levels of serum CRP,IL-1β,IL-6 and electrocardiogram waveform were compared between the two groups.The expression levels of serum CRP,IL-1β,IL-6 and electrocardiogram waveform were compared between the two groups.The correlation between the expression levels of serum CRP,IL-1βand IL-6 and electrocardiogram waveform after myocardial ischemia in coronary heart disease was analyzed.To determine the value of CRP,IL-1β,IL-6 and abnormal ECG waveform in predicting myocardial ischemia in coronary heart disease.Results:The levels of CRP(75.18±3.88)mg/L,IL-1β(32.40±4.04)ng/L and IL-6(49.32±7.58)pg/ml in the observation group were higher than those in the control group(54.64±3.52)mg/L,(14.87±2.95)ng/L and(105.21±7.05)pg/ml,the difference was significant(t=22.523,P=0.000,t=20.131,P=0.000,t=24.478,P=0.000).There were no significant differences in RR interval,T-wave duration and PR interval between the two groups(P>0.05).The QRS duration of the observation group was lower than that of the control group(t=2.064,P=0.043).QRS duration limit was adversely linked with CRP,IL-1β,and IL-6 levels(r=-0.541,P=0.005,r=-0.614,P=0.000,r=-0.592,P=0.001).The expression levels of serum CRP,IL-1β,IL-6 and QRS combined with the duration of QRS had a good predictive value for coronary heart disease myocardial ischemia.
