人工真皮在修复手指Ⅲ度氢氟酸烧伤中的临床应用

摘　　要：目的 探讨人工真皮在手指氢氟酸(hydrofluoric acid,HFA)深度烧伤中的临床应用效果。方法 回顾性分析自2021年7月至2022年7月,苏北人民医院烧伤整形科收治的10例(27指)Ⅲ度HFA手指烧伤患者。烧伤后1周待坏死组织界限清晰后对患指进行清创及人工真皮植入术,术后规律换药并观察人工真皮感染及血管化情况,2~3周后人工真皮硅胶膜分离,其下方创面自行愈合。结果 10例(27指)人工真皮修复,创面后期未植皮,全部自行愈合,无感染、破溃等并发症发生。术后平均随访12个月(6~15个月),患指外形美观,弹性良好,无臃肿、凹陷或明显瘢痕以及色素沉着;手指功能良好,无明显感觉障碍。结论 手指小面积深度HFA烧伤患者采用人工真皮修复术,无需行皮瓣转移或二次植皮即可修复创面,外观及功能均恢复良好。Objective To explore the clinical application effect of artificial dermis in the treatment of deep hydrofluoric acid burns on fingers. Methods A total of 10 patients with 27 fingers of third-degree hydrofluoric acid burns were admitted to Department of Burn and Plastic Surgery, Northern Jiangsu People's Hospital from July 2021 to July 2022. One week after the burn, the affected fingers were debrided and artificial dermis was implanted after the boundaries of the necrotic tissue were clear. After the operation, the dressing was changed regularly and the infection and vascularization of the artificial dermis were observed. After 2 to 3 weeks, the artificial dermis silicone membrane was separated, and the wound underneath healed spontaneously. Results A total of 27 fingers of 10 cases were treated with artificial dermis. No skin grafting was performed on the wound surface at the later stage. All wounds healed spontaneously without infection or other complications. The average follow-up period was 12 months(6 to 15 months) after surgery. The affected fingers had beautiful appearance, good elasticity, no swelling, depression, obvious scars or pigmentation;and normal function without obvious sensory impairment. Conclusion Artificial dermis repair is used for patients with small-area deep hydrofluoric acid burns on their fingers. The wound surface can be repaired without flap transfer or secondary skin grafting, and both the appearance and function are restored well.
