防旋股骨近端髓内钉与InterTan髓内钉在老年股骨粗隆间骨折中的应用

摘　　要：目的比较防旋股骨近端髓内钉(proximal femoral nail antirotation,PFNA)与InterTan髓内钉在老年股骨粗隆间骨折(intertrochanteric fracture of femur,IFOF)中的应用效果及对髋关节功能的影响。方法选取2019年12月—2022年12月齐鲁医药学院附属新泰市人民医院收治的128例老年IFOF患者为研究对象,按照治疗方式的不同将其分为对照组与研究组,各64例。对照组接受PFNA内固定治疗,研究组接受InterTan髓内钉内固定治疗。比较两组术中与术后情况、Harris评分与并发症发生情况。结果两组手术及骨折愈合时间时间、术中失血量比较,差异均无统计学意义(P均>0.05);与对照组相比,术后6个月时股骨颈缩短距离研究组更短,差异有统计学意义(P<0.05)。术后6个月,研究组Harris评分为(80.65±5.52)分,高于对照组的(75.65±6.00)分,差异有统计学意义(t=4.906,P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论与PFNA相比,InterTan髓内钉可以避免老年IFOF患者股骨颈缩短,且可促进髋关节功能的恢复。Objective To compare the effect of proximal femoral nail antirotation(PFNA)and InterTan intramedullary nail on intertrochanteric fracture of femur(IFOF)in elderly patients and its effect on hip joint function.Methods A total of 128 elderly IFOF patients admitted to Xintai People's Hospital Affiliated to Qilu Medical College from December 2019 to December 2022 were selected as the research objects.According to the different treatment methods,they were divided into the control group and the study group,with 64 cases in each group.The control group was treated with PFNA internal fixation,and the study group was treated with InterTan intramedullary nail internal fixation.The intraoperative and postoperative conditions,Harris score and complications were compared between the two groups.Results There were no significant differences in operation time,fracture healing time and intraoperative blood loss between the two groups(all P>0.05).Compared with the control group,the femoral neck shortening distance in the study group was shorter at 6 months after operation,and the difference was statistically significant(P<0.05).Six months after operation,the Harris score of the study group was(80.65±5.52)points,which was higher than(75.65±6.00)points of the control group,and the difference was statistically significant(t=4.906,P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Compared with PFNA,InterTan intramedullary nail can avoid shortening of femoral neck and promote the recovery of hip joint function in elderly patients with IFOF.
