摘 要:目的探讨磁共振扩散张量成像在腰椎间盘突出症患者神经根受压评估中的应用价值。方法选择2022年8月—2023年8月邹城市人民医院收治的53例腰椎间盘突出症患者作为腰椎间盘突出症组,同期选取53例健康志愿者作为对照组,均实施磁共振扩散张量成像检查,测量受压神经根和正常神经根的磁共振扩散张量成像参数,并比较两组的表观扩散系数(apparent diffusion coefficient,ADC)值和各向异性分数(fraction anisotropy,FA)。绘制受试者操作特征(receiver operating characteristic,ROC)曲线,分析磁共振扩散张量成像在腰椎间盘突出症患者神经根受压评估中的应用价值。结果腰椎间盘突出症组患者受压神经根的ADC值高于对照组,FA值低于对照组,差异均有统计学意义(P均<0.05)。ROC曲线显示,磁共振扩散张量成像ADC值与FA值评估腰椎间盘突出症患者神经根受压的曲线下面积(area under the curve,AUC)分别为0.906、0.863,联合ADC值与FA值的AUC为0.913,灵敏度、特异度分别为92.50%、81.10%。结论磁共振扩散张量成像在腰椎间盘突出症患者神经根受压评估中具有一定的应用价值,磁共振扩散张量成像参数ADC值与FA值均能获得较高的灵敏度和特异度,且两者联合诊断价值更高。Objective To explore the application value of magnetic resonance diffusion tensor imaging in the evaluation of nerve root compression in patients with lumbar disc herniation.Methods From August 2022 to August 2023,a total of 53 patients with lumbar disc herniation admitted to Zoucheng People's Hospital were selected as the lumbar disc herniation group,and 53 healthy volunteers in the same period were selected as the control group.Magnetic resonance diffusion tensor imaging was performed to measure the magnetic resonance diffusion tensor imaging parameters of compressed nerve roots and normal nerve roots.The apparent diffusion coefficient(ADC)value and fraction anisotropy(FA)of the two groups were compared.The receiver operating characteristic(ROC)curve was drawn to analyze the application value of magnetic resonance diffusion tensor imaging in the evaluation of nerve root compression in patients with lumbar disc herniation.Results The ADC value of the compressed nerve root in the lumbar disc herniation group was higher than that in the control group,and the FA value was lower than that in the control group,the differences were statistically significant(both P<0.05).ROC curve showed that the area under the curve(AUC)of ADC value and FA value of magnetic resonance diffusion tensor imaging in evaluating nerve root compression in patients with lumbar disc herniation were 0.906 and 0.863,respectively.The AUC of combined ADC value and FA value was 0.913,and the sensitivity and specificity were 92.50% and 81.10%, respectively. Conclusion Magnetic resonance diffusion tensor imaginghas certain application value in the evaluation of nerve root compression in patients with lumbar disc herniation.The ADC value and FA value of magnetic resonance diffusion tensor imaging parameters can obtainhigher sensitivity and specificity, and the combination of the two has higher diagnostic value.
