摘 要:目的 探究使用托吡卡胺进行散瞳前后的瞳孔变化(自然瞳孔、瞳孔散大8 mm)对可视化角膜生物力学分析仪(Corvis ST)测量指标结果的影响因素。方法 选取2024年3—10月于宜昌爱尔眼科医院屈光手术专科就诊的患者32例(64眼)为研究对象,对同一患者(2只眼)进行2次同一检查操作,第一次为自然瞳孔,第二次为用托吡卡胺进行散瞳后(瞳孔散大8 mm)。比较两次患者的角膜生物力学检查结果,对其影响因素进行分析,包括角膜地形图生物力学指数(tomographic biomechanical index,TBI)、Corvis生物力学指数(Corvis biomechanical index,CBI)、角膜地形图的Belin/Ambrosio综合偏差值(Belin/Ambrosio deviation display-deviation,BAD-D)、硬度参数(stiffness parameter applanation1,SPA1)、综合半径(integratedradius,IR)、最薄点厚度/厚度变化率(ambrósiorelationalthickness,ARTh)、最大形变幅度(deformation amplitude,DA)、眼压(intraocular pressure,IOP)。结果 自然瞳孔下、瞳孔散大8 mm的TBI、CBI、BAD-D、SPA1、IR、ARTh、DA、IOP比较,差异均无统计学意义(P均>0.05)。用托吡卡胺进行散瞳前后的不同瞳孔情况不是TBI(OR=0.814,P=0.870)、CBI(OR=0.294,P=0.328)、BAD-D(OR=1.078,P=0.899)、SPA1(OR=0.980,P=0.224)、IR(OR=0.747,P=0.446)、ARTh(OR=0.999,P=0.770)、DA(OR=2.250,P=0.422)、IOP(OR=1.193,P=0.081)指标的影响因素。结论 患者用托吡卡胺进行散瞳前的自然瞳孔、进行散瞳后(瞳孔散大8 mm)的角膜生物力学检查结果相似。Objective To explore the influencing factors of pupil changes(natural pupil,mydriasis 8 mm) before and after mydriasis with tropicamide on the results of visual corneal biomechanical analyzer(Corvis ST).Methods A total of 32 patients(64 eyes) who underwent refractive surgery in Yichang Aier Eye Hospital from March to October 2024 were selected as the study subjects.The same patient(2 eyes) underwent the same examination twice.The first time was natural pupil,and the second time was after mydriasis with tropicamide(mydriasis 8 mm).The results of corneal biomechanical examination were compared between the two groups,and the influencing factors were analyzed,including tomographic biomechanical index(TBI),corvis biomechanical index(CBI),corneal topography Belin/Ambrosio deviation display-deviation(BAD-D),stiffness parameter applanation 1(SPA1),integrated radius(IR),ambrósio relational thickness(ARTh),deformation amplitude(DA),intraocular pressure(IOP).Results There were no significant differences in TBI,CBI,BAD-D,SPA1,IR,ARTh,DA and IOP under natural pupil and 8 mm pupil dilation(all P>0.05).Different pupil conditions before and after mydriasis with tropicamide were not the influencing factors of TBI(OR=0.814,P=0.870),CBI(OR=0.294,P=0.328),BAD-D(OR=1.078,P=0.899),SPA1(OR=0.980,P=0.224),IR(OR=0.747,P=0.446),ARTh(OR=0.999,P=0.770),DA(OR=2.250,P=0.422),IOP(OR=1.193,P=0.081).Conclusion The results of corneal biomechanical examination before and after mydriasis(mydriasis 8 mm) were similar.
