化肝解毒汤联合恩替卡韦治疗慢性乙型肝炎肝硬化的临床效果

摘　　要：目的探讨慢性乙型肝炎(chronic hepatitis B,CHB)肝硬化患者采用化肝解毒汤联合恩替卡韦治疗的临床效果。方法选取2020年8月—2023年8月甘肃省宁县人民医院收治的80例CHB肝硬化患者,根据不同的治疗方法分成对照组(n=40,给予恩替卡韦治疗)和研究组(n=40,给予化肝解毒汤联合恩替卡韦治疗)。比较两组临床疗效、T淋巴细胞亚群、肝纤维化指标及不良反应发生情况。结果研究组治疗总有效率[97.50%(39/40)]较对照组更高[80.00%(32/40)],差异有统计学意义(χ^(2)=4.507,P<0.05)。治疗后,与对照组相比,研究组CD3^(+)、CD4^(+)更高,CD8^(+)、Ⅲ型前胶原、Ⅳ型胶原、透明质酸、层黏连蛋白水平更低,差异均有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论CHB肝硬化患者采用化肝解毒汤联合恩替卡韦治疗,能够提升临床疗效,促进T淋巴细胞亚群以及肝纤维化状况改善,且安全性较高。Objective To investigate the clinical effect of Huagan Jiedu Decoction combined with entecavir in the treatment of patients with chronic hepatitis B (CHB) cirrhosis.Methods From August 2020 to August2023,80 patients with CHB cirrhosis admitted to People's Hospital of Ningxian in Gansu Province were selected and divided into control group (n=40,treated with entecavir) and study group (n=40,treated with Huagan Jiedu Decoction combined with entecavir) according to different treatment methods.The clinical efficacy,T lymphocyte subsets,liver fibrosis indexes and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the study group[97.50%(39/40)]was higher than[80.00%(32/40)]in the control group,and the difference was statistically significant (χ~2=4.507,P<0.05).After treatment,compared with the control group,the levels of CD3~+and CD4~+in the study group were higher,and the levels of CD8~+,typeⅢprocollagen,type IV collagen,hyaluronic acid and laminin were lower,the differences were statistically significant (all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Patients with CHB cirrhosis treated with Huagan Jiedu Decoction combined with entecavir can promote the improvement of clinical efficacy,promote the improvement of T lymphocyte subsets and liver fibrosis,and have high safety.
