自拟开癃通闭汤联合盐酸坦索罗辛在良性前列腺增生患者经尿道双极等离子电切术后的应用

摘　　要：目的 探讨良性前列腺增生患者经尿道双极等离子电切术后应用自拟开癃通闭汤联合盐酸坦索罗辛的效果。方法 选取2021年1月—2023年12月江苏省兴化市中医院泌尿外科接收的120例良性前列腺增生患者,统一实施经尿道双极等离子电切术治疗,按治疗方法不同分为对照组(盐酸坦索罗辛治疗)和观察组(盐酸坦索罗辛联合自拟开癃通闭汤治疗),每组60例,比较两组治疗总有效率、性激素水平、前列腺相关情况、前列腺症状评分、术后并发症发生率。结果 观察组患者的治疗总有效率高于对照组[91.67%(55/60) vs 78.33%(47/60)],差异有统计学意义(χ^(2)=4.183,P<0.05)。治疗后,睾酮、血浆雌二醇以及黄体生成素水平及前列腺体积、膀胱残余尿量、最大尿流率、血清前列腺特异性抗原两组对比,观察组均优于对照组,差异均有统计学意义(P均<0.05)。治疗后,与对照组相比,国际前列腺症状评分及术后并发症发生率观察组均更低,差异均有统计学意义(P均<0.05)。结论 自拟开癃通闭汤联合西药的治疗方式对于实施了经尿道双极等离子电切术的良性前列腺增生患者有着良好的应用效果。Objective To explore the effect of self-made Kailong Tongbi Decoction combined with tamsulosin hydrochloride in patients with benign prostatic hyperplasia after transurethral bipolar plasma electrocision.Methods A total of 120 patients with benign prostatic hyperplasia received by the Department of Urology,in Xinghua Hospital of Traditional Chinese Medicine from January 2021 to December 2023 were selected and treated with transurethral bipolar plasma electrocision.According to different treatment methods,they were divided into control group(tamsulosin hydrochloride) and observation group(tamsulosin hydrochloride combined with self-made Kailong Tongbi Decoction),with 60 cases in each group.The total effective rate of treatment,sex hormone levels,prostate-related conditions,prostate symptom score and incidence of postoperative complications were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group [91.67%(55/60) vs 78.33%(47/60)],and the difference was statistically significant(χ~2=4.183,P<0.05).After treatment,the levels of testosterone,plasma estradiol and luteinizing hormone,prostate volume,bladder residual urine volume,maximum urinary flow rate and serum prostate specific antigen in the observation group were better than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,compared with the control group,the international prostate symptom score and the incidence of postoperative complications were lower in the observation group,and the differences were statistically significant(both P<0.05).Conclusion The treatment of Self-made Kailong Tongbi Decoction combined with Western medicine has a good application effect on patients with benign prostatic hyperplasia who have undergone transurethral bipolar plasma electrocision.
