内服龙胆泻肝汤、外敷金黄散在湿热内蕴型丹毒治疗中的临床研究

摘　　要：目的 探讨湿热内蕴型丹毒应用龙胆泻肝汤内服、金黄散外敷的临床效果。方法 选择2021年9月—2024年4月徐州市铜山区中医院普外科接诊的52例下肢丹毒患者,按照治疗方法不同分成西医组与中西医组,每组26例。西医组予以哌拉西林、左氧氟沙星治疗,中西医组在西医组用药基础上,予以龙胆泻肝汤内服加金黄散外敷治疗。对比两组临床疗效、中医证候积分、症状改善情况。结果 中西医组的治疗总有效率为96.15%(25/26),与西医组的80.77%(21/26)相比,差异无统计学意义(χ^(2)=3.015,P>0.05)。治疗2周后,两组局部症状及体征、发热程度、口干咽燥、舌苔评分均比治疗前下降,且中西医组低于西医组,差异均有统计学意义(P均<0.05)。治疗2周后,两组口腔温度、皮肤温度、肤色、水肿程度评分均比治疗前下降,且中西医组低于西医组,差异均有统计学意义(P均<0.05)。结论 湿热内蕴型丹毒应用龙胆泻肝汤内服加金黄散外敷,可显著改善患者发热、口干咽燥等症状,减轻下肢水肿程度,促进下肢肤色恢复正常。Objective To explore the clinical effect of oral administration of Longdan Xiegan Decoction and external application of Jinhuang Powder in the treatment of erysipelas with endoretention of damp-heat syndrome.Methods From September 2021 to April 2024,52 patients with lower extremity erysipelas admitted to the Department of General Surgery of Xuzhou Tongshan District Hospital of Traditional Chinese Medicine were selected and divided into Western medicine group and traditional Chinese and Western medicine group according to different treatment methods,with 26 cases in each group.The Western medicine group was treated with piperacillin and levofloxacin,and the traditional Chinese and Western medicine group was treated with oral administration of Longdan Xiegan Decoction and external application of Jinhuang Powder on the basis of the Western medicine group.The clinical efficacy,traditional Chinese medicine syndrome scores and symptom improvement of the two groups were compared.Results The total effective rate of the traditional Chinese and Western medicine group was 96.15%(25/26),compared with 80.77%(21/26) in the Western medicine group,and the difference was not statistically significant(χ~2=3.015,P>0.05).After 2 weeks of treatment,the scores of local symptoms and signs,fever degree,dry mouth and throat,and tongue coating in the two groups were lower than those before treatment,and the scores in the traditional Chinese and Western medicine group were lower than those in the Western medicine group,and the differences were statistically significant(all P<0.05).After 2 weeks of treatment,the scores of oral temperature,skin temperature,skin color and edema degree in the two groups were lower than those before treatment,and the scores in the traditional Chinese and Western medicine group were lower than those in the Western medicine group,and the differences were statistically significant(all P<0.05).Conclusion The effect of oral administration of Longdan Xiegan Decoction and external application of Jinhuang Powder on er
