腹部超声联合直肠超声诊断青春期女性附件扭转的价值

摘　　要：目的 分析腹部超声联合直肠超声在青春期女性附件扭转中的诊断价值。方法 选择2020年3月—2023年3月枣庄市妇幼保健院接诊的80例疑似青春期女性附件扭转患者,分别提供直肠超声诊断、腹部超声诊断和联合诊断,以手术探查作为金标准,分析不同诊断方法对青春期女性附件扭转的诊断价值。结果 手术探查结果显示70例阳性、10例阴性;直肠超声检出62例阳性、18例阴性;腹部超声检出60例阳性、20例阴性;联合诊断共检出69例阳性、11例阴性。联合诊断的灵敏度为97.14%(68/70)、准确度为96.25%(77/80),均高于直肠超声的85.71%(60/70)、85.00%(68/80)以及腹部超声的81.43%(57/70)、80.00%(64/80),差异均有统计学意义(χ^(2)=7.361、9.853,P均<0.05)。3种检查方法的特异度比较,差异无统计学意义(P>0.05)。结论 青春期女性附件扭转诊断中,运用腹部超声联合直肠超声诊断具有较高的诊断效能。Objective To analyze the diagnostic value of abdominal ultrasound combined with rectal ultrasound analysis in adolescent female adnexal torsion.Methods From March 2020 to March 2023,80 patients with suspected adolescent female adnexal torsion were selected from Zaozhuang Maternal and Child Health Hospital.Rectal ultrasound diagnosis,abdominal ultrasound diagnosis and combined diagnosis were provided respectively.Surgical exploration was used as the gold standard to analyze the diagnostic value of different diagnostic methods for adolescent female adnexal torsion.Results The results of surgical exploration showed that 70cases were positive and 10 cases were negative.62 positive cases and 18 negative cases were detected by rectal ultrasound.Abdominal ultrasound detected 60 cases of positive and 20 cases of negative,a total of 69 positive cases and 11 negative cases were detected by combined diagnosis.The sensitivity and accuracy of combined diagnosis were 97.14%(68/70) and 96.25%(77/80),which were higher than 85.71%(60/70) and 85.00%(68/80)of rectal ultrasound and 81.43%(57/70) and 80.00%(64/80) of abdominal ultrasound,and the differences were statistically significant(χ~2=7.361,9.853,both P<0.05).There was no significant difference in the specificity of the three methods(P>0.05).Conclusion Abdominal ultrasound combined with rectal ultrasound has high diagnostic efficiency in the diagnosis of adnexal torsion in adolescent women.
