多层螺旋CT与彩色多普勒超声对非外伤性急腹症的诊断效果比较

摘　　要：目的 比较多层螺旋CT(multi-slice spiral CT,MSCT)与彩色多普勒超声(color Doppler ultrasound,CDU)对非外伤性急腹症的诊断效果。方法 选取2022年2月—2024年1月新泰市人民医院收治的疑似非外伤性急腹症患者共计95例,以临床随诊、手术探查及术后病理检查为金标准,所有患者均行MSCT、CDU检查,比较MSCT、CDU单独及联合检查的诊断结果,并分析诊断效能,比较不同检查方式对病变的诊断符合率。结果 95例疑似非外伤性急腹症患者中,经临床随诊、手术探查及术后病理检出阳性74例,阴性21例。MSCT检出阳性67例,阴性28例,kappa=0.700;CDU检出阳性64例,阴性31例,kappa=0.269;联合检查检出阳性73例,阴性22例,kappa=0.910。联合检查的灵敏度、特异度、阳性预测值、阴性预测值、准确度均高于MSCT、CDU检查,差异均有统计学意义(P均<0.05)。MSCT、CDU单独及联合检查对急性胰腺炎、泌尿系统结石、急性阑尾炎、急性胆囊炎、机械性肠梗阻病变率的诊断符合率比较,差异均无统计学意义(P均>0.05)。结论 对非外伤性急腹症进行检查时,MSCT与CDU联合检查具有较高的诊断价值。Objective To compare the diagnostic effect of multi-slice spiral CT(MSCT) and color Doppler ultrasound(CDU) on non-traumatic acute abdomen.Methods A total of 95 patients with suspected nontraumatic acute abdomen admitted to Xintai People's Hospital from February 2022 to January 2024 were selected.Clinical follow-up,surgical exploration and postoperative pathological examination were used as the gold standard.All patients were examined by MSCT and CDU.The diagnosis results of MSCT,CDU alone and in combination were compared,the diagnostic efficacy was analyzed,and the diagnostic coincidence rate of lesions in different examination methods was compared.Results Among 95 patients with suspected nontraumatic acute abdomen,74 cases were positive and 21 cases were negative by clinical follow-up,surgical exploration and postoperative pathology.MSCT detected 67 positive cases and 28 negative cases,kappa=0.700;CDU detected 64 positive cases and 31 negative cases,kappa=0.269;the combined examination detected 73positive cases and 22 negative cases,kappa=0.910.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of combined examination were higher than those of MSCT and CDU,and the differences were statistically significant(all P<0.05).There were no significant differences in the diagnostic coincidence rates of MSCT,CDU alone and in combination in acute pancreatitis,urinary calculi,acute appendicitis,acute cholecystitis and mechanical intestinal obstruction(all P>0.05).Conclusion MSCT combined with CDU has high diagnostic value in the examination of non-traumatic acute abdomen.
