揿针联合枸地氯雷他定治疗IgE升高型慢性荨麻疹42例临床研究

摘　　要：目的 观察揿针联合枸地氯雷他定治疗IgE升高型慢性荨麻疹的临床疗效。方法 选取2021年7月至2022年7月南充市中心医院皮肤科门诊诊断符合IgE升高型慢性荨麻疹的84例患者作为受试者,采用简单随机法分为治疗组42例和对照组42例。2组患者均口服枸地氯雷他定片治疗,治疗组患者在此基础上联合揿针治疗。比较2组临床疗效及治疗前后7日荨麻疹活动度评分(UAS7)、血清干扰素-γ(IFN-γ)、总免疫球蛋白E(IgE)、白细胞介素-4(IL-4)含量及不良反应。治疗结束1个月后随访,评估2组的复发率。结果 治疗组有效率(83.33%)明显高于对照组(53.76%)(P.<0.05)。治疗后,2组UAS7、IL-4、总IgE均明显降低(P.<0.01),且治疗组较对照组更低(P.<0.05)。治疗后2组IFN-γ均升高(P.<0.01),且治疗组较对照组更高(P.<0.05)。治疗中,治疗组出现头晕4例,上腹不适1例。对照组出现头晕3例。治疗组复发率(17.14%)明显低于对照组(43.48%)。结论 揿针联合枸地氯雷他定治疗IgE升高型慢性荨麻疹疗效确切,可改善患者的免疫状态。Objective To explore the clinical eff cacy of press needles combined with desloratadine citrate disodium in the treatment of chronic urticaria with elevated IgE.Methods A total of 84 patients diagnosed with chronic urticaria with elevated IgE in department of dermatology of Nanchong Central Hospital from July 2021 to July 2022 were selected as subjects and then randomly divided into a treatment group with 42 cases and a control group with 42 cases.All patients were orally administered desloratadine citrate disodium tablets for treatment.The 42 cases in the treatment group were additionally treated with press needles.The clinical efficacy,the 7-day Urticaria Activity Score(UAS7),the levels of interferon-(IFN-),immunoglobulin E(IgE),and interleukin-4(IL-4)in the serum,as well as the adverse reactions were evaluated and observed between the two groups before and after the treatment.Finally,one month after the end of the treatment,a follow-up was conducted to evaluate the recurrence rates o f the two groups.Results The effective rate of the treatment group(83.33%)was significantly higher than that of the control group(53.76%)(P<0.05).Four weeks after the treatment,the scores of UAS7,and the serum levels of IL-4 and IgE in both groups decreased significantly(P<0.01),and the values in the treatment group were even lower than those in the control group(P<0.05).After the treatment,the serum level of IFN-y increased in both groups(P<0.01),and the level in the treatment group was higher than that in the control group(P<0.05).In the treatment group,4 cases experienced dizziness and 1 case had discomfort in the upper abdomen,In the control group,3 cases presented with dizziness.The recurrence rate of the treatment group(17.14%)was notably lower than that of the control group(43.48%).Conclusion The combination of press needles and desloratadine citrate disodium has a definite curative effect in the treatment of chronic urticaria with elevated IgEand can improve the immune status of patients.
