医用皮肤护理软膏对微等离子射频治疗增生性瘢痕术后创面愈合的影响

摘　　要：目的 观察医用皮肤护理软膏对微等离子射频(MPRF)治疗增生性瘢痕(HTS)术后创面愈合的效果。方法 选取2022年5月至2023年5月中国人民解放军总医院第四医学中心烧伤整形医学部收治的60例HTS患者,将患者自身治疗区域HTS分为2个治疗区(A、B),A区仅行MPRF治疗,B区行MPRF治疗后创面外用医用皮肤护理软膏,共治疗5次,每次治疗间隔1.5个月。比较A、B区开始治疗前及第5次治疗3个月后温哥华瘢痕评定量表(VSS)评分,并比较A、B区临床疗效、第1次治疗后痂皮完全自然脱落时间及不良反应发生情况。结果 第5次治疗3个月后,A区和B区VSS评分均明显下降(P<0.05),且B区VSS评分低于A区(P<0.05)。A区有效率为85%(51/60),B区有效率为95%(57/60),B区有效率明显高于A区(P<0.05)。第1次治疗后B区痂皮完全自然脱落时间较A区短(P<0.05)。B区不良反应发生率明显低于A区(P<0.05),持续性红斑持续时间较A区短(P<0.05),疼痛程度较A区轻(P<0.05)。结论 医用皮肤护理软膏用于MPRF治疗HTS术后创面,有助于促进创面愈合,增强MPRF治疗HTS的效果,减少不良反应的发生,缓解疼痛。Objective To evaluate the efficacy of medical skin care ointment in promoting wound healing after microplasma radiofrequency(MPRF)treatment for hypertrophic scars(HTS).Methods Sixty HTS patients treated at the Department of Burns and Plastic Surgery,Fourth Medical Center of the General Hospital of the Chinese People's Liberation Army were enrolled.Each patient's HTS was divided into two treatment areas(A and B).Area A received MPRF alone,while Area B received MPRF followed by topical application of medical skin care ointment.Treatments were administered five times,with 1.5 months between sessions.Vancouver Scar Scale(VsS)scores were compared between areas A and B before treatment and 3 months after the fth session.Clinical efficacy,time to complete natural crust shedding after the first treatment,and adverse reactions were also analyzed.Results Three months after the fifth treatment,VSS scores significantly decreased in both areas(P<0.05),with Area B showing a greater reduction than Area A.The overall response rate was 85%(51/60)in Area A and 95%(57/60)in Area B,with a statistically significant difference(P<0.05).Area B demonstrated shorter crust shedding time(P<0.05),lower incidence of adverse reactions(P<0.05),shorter duration of persistent erythema(P<0.05),and less severe pain(P<0.05)compared to Area A.ConclusionTopical medical skin care ointment following MPRF treatment for HTS promotes wound healing,enhances therapeutic outcomes,reduces adverse reactions,adverse reactions,and alleviates pain.
