部分单侧雀斑样痣1例

摘　　要：患者女性,26岁。左侧面部散在黄褐色色素沉着斑20余年。皮肤科检查显示:色素沉着斑大小不一,呈不规则形,部分融合成片,沿面中线左侧呈单侧散在分布,边界清晰,与面中线右侧对比明显。反射式共聚焦激光扫描显微镜检查发现,皮损处基底层色素明显增多。组织病理检查显示表皮改变不明显,基底层可见增生的黑素细胞,局灶呈雀斑样分布,真皮浅层血管周围少量淋巴细胞浸润。结合各项检查,诊断为部分单侧雀斑样痣。患者拒绝接受激光治疗,遂予维生素C和疏肝活血颗粒治疗,目前仍在随访中。A 26-year-old female presented with scattered yellowish-brown hyperpigmented macules on the left hemifacial region for over 20 years:Dermatological examination revealed multiple irregularly shaped pigmented lesions of varying sizes,partially coalescing into patches.These lesions exhibited unilateral distribution along the left facial midline with sharply demarcated borders,demonstrating striking asymmetry compared to the unaffected right side.Confocal laser scanning microscopy examination showed a notable increase in pigmentation in the basal layer of the affected skin.Histopathological examination indicateed that changes in the epidermis were minimal,but proliferating melanocytes were visible in the basal layer,with a focal,freckle-like distribution.There was mild infiltration of lymphocytes around superficial dermal blood vessels.Based on the clinical and examination findings,the patient was finally diagnosed as partial unilateral lentiginosis.The patient declined laser intervention and was instead treated with vitamin C and Shugan Huoxue Granule.The patient is currently being followed up.
