摘 要:目的探讨超微血流成像技术对跖背动脉血流评估在2型糖尿病周围神经病变(DPN)诊断中的价值。方法选取糖尿病患者64例,将其分为周围神经病变组(n=38)与单纯糖尿病组(n=26),选取同期健康体检者24例作为对照组;收集3组受试者一般临床资料,对所有受试者行超微血流成像技术检查,观察并记录其跖背动脉血流情况;通过ROC曲线分析确定相关诊断指标的效能。结果跖背动脉管径及PSV在3组间两两比较均有统计学意义(P<0.05);跖背动脉管径、糖尿病病程、糖化血红蛋白、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)是DPN发生的独立危险因素;跖背动脉管径单独对DPN预测的AUC为0.725,灵敏度为0.923,特异度为0.526,联合糖尿病病程及TG对DPN的预测AUC为0.845,灵敏度为0.789,特异度为0.808,具有较好的诊断效能。结论超微血流成像对跖背动脉血流的评估可作为周围神经病变诊断的有效辅助手段。Objective To evaluate the value of blood flow assessment of dorsal metatarsal artery by superb microvascular imaging(SMI)in the diagnosis of type 2 diabetic peripheral neuropathy.Methods A total of 64 patients with diabetes were divided into peripheral neuropathy group(n=38)and simple diabetes group(n=26),and 24 healthy subjects during the same period were selected as control group.General clinical data of the 3 groups were collected,all subjects were examined by SMI technology,and the blood flow of the dorsal metatarsal artery in the 3 groups was observed and recorded.The efficiency of relevant diagnostic indexes was determined by ROC curve analysis.Results The pairwise comparison of dorsal metatarsal artery diameter and PSV among the three groups was statistically significant(P<0.05).Dorsal metatarsal artery diameter,diabetes course,glycosylated hemoglobin,TG and HDL-C were independent risk factors for DPN.The AUC for DPN prediction of dorsal metatarsal artery diameter alone was 0.725,the sensitivity was 0.923,and the specificity was 0.526.The AUC for DPN prediction of combined diabetes course and TG was 0.845,the sensitivity was 0.789,and the specificity was 0.808,showing good diagnostic efficacy.Conclusion The blood flow assessment of the dorsal metatarsal artery by SMI can be an effective auxiliary means for the diagnosis of peripheral neuropathy.
