内科胸腔镜在不明原因胸膜疾病诊断中的价值探讨

摘　　要：目的探讨不明原因胸膜疾病的一般临床特征、病理类型和内科胸腔镜下表现,以及内科胸腔镜对不明原因胸膜疾病的诊断价值。方法分析81例行内科胸腔镜检查的不明原因胸膜疾病患者的资料,根据病理结果将其分为良性胸膜疾病组(n=52)和恶性胸膜疾病组(n=29),分析比较2组患者的临床特征和内科胸腔镜下表现,计算内科胸腔镜检查对不明原因胸膜疾病的诊断率。结果不明原因胸膜疾病以结核性胸膜炎和肺癌胸膜转移为多见,81例通过内科胸腔镜确诊73例,内科胸腔镜诊断阳性率为90.12%。结论不明原因胸膜疾病以结核性和肿瘤性为多见。内科胸腔镜检查在不明原因胸膜疾病中有诊断率高、安全性高和创伤小的优点,是胸膜疾病诊断的重要手段。Objective To explore the general clinical features,pathological types and thoracoscopic manifestations of unexplained pleural diseases,as well as the diagnostic value of medical thoracoscopy in unexplained pleural diseases.Methods 99 patients with unexplained pleural diseases who underwent internal thoracoscopy were analyzed,of which 81 cases were mostly tuberculous and neoplastic.The patients were divided into benign pleural disease group(n=52)and malignant pleural disease group(n=29)according to the pathological findings,and the clinical features and internal thoracoscopic manifestations of the two groups were analyzed and compared to calculate the diagnostic rate of unexplained pleural disease by internal thoracoscopy.Results The most common pleural diseases of unknown causes were tuberculous pleurisy and pleural metastasis of lung cancer.99 patients with unexplained pleural diseases were finally diagnosed in 81 cases,with a diagnostic rate of 81.82%,among which 73 cases were diagnosed by internal thoracoscopy,and the diagnostic positivity rate of internal thoracoscopy was 90.12%.Conclusion The most common pleural diseases of unknown causes are tuberculosis and tumor.Medical thoracoscopy has the advantages of high diagnosis rate,high safety and less trauma in unexplained pleural diseases,and it is an important means for the diagnosis of pleural diseases.
