摘 要:目的探讨幽门螺杆菌(Helicobacter pylori,Hp)阳性的慢性萎缩性胃炎患者胃黏膜组织miR-34a和叉头框转录因子D3(FOXD3)表达特点及临床意义。方法以92例Hp阳性的慢性萎缩性胃炎患者为研究对象,胃镜检查时采集萎缩胃黏膜组织(病变胃黏膜组织)和周边正常形态的胃黏膜组织(正常胃黏膜组织)。RT-PCR检测组织标本中miR-34a和FOXD3 mRNA表达水平;Western blot检测组织标本中FOXD3蛋白表达水平。进行1 a随访,记录患者预后情况,构建ROC曲线,分析miR-34a、FOXD3表达对患者预后的预测价值。结果Hp阳性的慢性萎缩性胃炎患者病变胃黏膜组织中miR-34a表达水平低于正常胃黏膜组织(P<0.01),FOXD3 mRNA、FOXD3蛋白表达水平高于正常胃黏膜组织(P<0.05)。重度患者miR-34a表达水平低于中度和轻度患者,FOXD3 mRNA、FOXD3蛋白表达水平高于中度和轻度患者(P<0.05);中度患者miR-34a表达水平低于轻度患者,FOXD3 mRNA、FOXD3蛋白表达水平高于轻度患者(P<0.05)。预后不良的慢性萎缩性胃炎患者病变胃黏膜组织中miR-34a水平低于预后良好患者,FOXD3 mRNA、FOXD3蛋白表达水平高于预后良好患者(均P<0.05)。慢性萎缩性胃炎患者病情严重程度与病变胃黏膜组织中miR-34a表达呈负相关关系(P<0.05),与FOXD3 mRNA、FOXD3蛋白表达呈正相关关系(P<0.05);预后水平与病变胃黏膜组织中miR-34a表达呈正相关关系(P<0.05),与FOXD3 mRNA、FOXD3蛋白表达呈负相关关系(P<0.05)。miR-34a、FOXD3 mRNA、FOXD3蛋白联合的预测价值最高(AUC=0.953)。结论Hp阳性的慢性萎缩性胃炎患者胃黏膜组织中miR-34a低表达、FOXD3高表达,且与病情严重程度和预后相关,联用对患者预后具有较高的预测价值。Objective To investigate the expression characteristics and clinical significance of miR-34a and forkhead box D3(FOXD3)in gastric mucosal tissues of Helicobacter pylori(Hp)positive chronic atrophic gastritis patients.Methods Ninety-two Hp positive chronic atrophic gastritis patients were studied,and atrophic gastric mucosal tissues(diseased gastric mucosal tissues)as well as peripheral normal gastric mucosal tissues(normal gastric mucosal tissues)were collected at the time of gastroscopy.miR-34a and FOXD3 mRNA expression levels were detected in tissue specimens by RT-PCR,and the protein expression level of FOXD3 was assessed in tissue specimens by Western blot.The patients were followed up for 1 a,and their prognosis was recorded.ROC curves were constructed to analyze the predictive value of miR-34a and FOXD3 expression on the patients’prognosis.Results The expression level of miR-34a was lower in atrophic gastric mucosal tissues of patients with Hp positive chronic atrophic gastritis compared to normal gastric mucosal tissues(P<0.01).In contrast,the expression levels of FOXD3 mRNA and FOXD3 protein were higher in atrophic gastric mucosal tissues than in normal gastric mucosal tissues(P<0.05).Among patients,those with severe gastritis exhibited lower miR-34a levels than those with moderate and mild gastritis,while FOXD3 mRNA and protein levels were higher in severe group(P<0.05).Additionally,the miR-34a expression level in moderate patients was lower than in mild patients,and the FOXD3 mRNA and protein levels were higher in moderate patients compared to mild patients(P<0.05).In atrophic gastric mucosal tissues of chronic atrophic gastritis patients with poor prognosis,miR-34a levels were lower,and FOXD3 mRNA and protein expression levels were higher than those in patients with good prognosis(all P<0.05).The severity of disease in patients with chronic atrophic gastritis was negatively correlated with miR-34a expression in atrophic gastric mucosal tissue(P<0.05)and positively correlated with FOXD3 mRNA and FOXD3
