吸入性烧伤致呼吸衰竭肠内营养患者留置鼻空肠管和鼻胃管的短期效果比较

摘　　要：目的 比较留置鼻空肠管、留置鼻胃管用于呼吸衰竭患者营养支持的效果。方法 回顾性分析2018年2月—2019年3月本院收治的104名呼吸衰竭患者的临床资料,根据其营养支持方法分为对照组(鼻胃管留置)和实验组(鼻空肠管留置)两组,每组各52例。比较两组患者3个时间点(干预前、干预3 d、干预7 d)10项指标(Hb、PA、ALB、IL8、IL-18、TNF-α、ESO、IgA、IgM、IgG)含量的统计学差异。比较两组患者并发症发生率的统计学差异。结果 重复测量方差分析,10项指标均有P_总<0.001、P_(组间)<0.001、P_(组内)<0.001,P_(交互)>0.001。组内比较,随时间推移,10项指标均有下降,差异均具有统计学意义(P<0.001)。各指标变化幅度进行组间比较,实验组Hb为(4.60±0.38)g/L,小于对照组的(15.60±3.42)g/L;实验组的ALB为(1.05±0.14)g/L,小于对照组的(8.01±2.02)g/L;实验组的PA为(0.03±0.01)g/L,小于对照组的(0.05±0.01)g/L;实验组的IL-8为(5.02±0.97)pg/ml,高于对照组的(3.40±0.73)pg/ml;实验组的TNF-α为(5.22±0.47)ng/L,高于对照组的(3.87±1.33)ng/L;实验组的EOS为(2.58±0.34)%,高于对照组的(2.37±0.40)%;实验组的IgA(0.23±0.03)g/L,低于对照组的(0.72±0.25)g/L;实验组的IgG(1.33±0.31)g/L,低于对照组的(5.28±0.74)g/L。结论 呼吸衰竭患者留置鼻空肠管进行营养支持,有助于改善营养状况、提升机体免疫功能、缓解全身炎症状态。Objective To compare the effect of indwelling nasojejunal tube and nasogastric tube for nutritional support in patients with respiratory failure.Methods A retrospective analysis was conducted on the clinical data of 104 patients with respiratory failure who admitted at our hospital from February 2018 to March 2019.Patients were divided into control group(nasogastric tube indwelling)and study group(nasal jejunal tube indwelling)based on the nutrition support method.The statistical differences of 10 indicators(Hb,PA,propagated,IL8,IL18,TNF alpha,ESO,IgA,IgM and IgG)content at three time point(before intervention,intervention intervention in 3 d,7d)were compared between two groups.The incidence of complications was compared between the two groups.Results 10 indicators showed that P(total)<0.001,P(inter-group)<0.001,P(intra-group)<0.001,P(interactive)>0.001 according to repeated measurement analysis of variance.10 indicators all decreased over time within the group comparison,and the differences were statistically significant(P<0.001).The change range of each indicator was compared between the groups.The Hb of the study group(4.60±0.38)g/L was less than that of the control group(15.60±3.42)g/L,the ALB of the study group(1.05±0.14)g/L was less than that of the control group(8.01±2.02)g/L,and the PA of the study group(0.03±0.01)g/L was less than that of the control group(0.05±0.01)g/L.IL8(5.02±0.97)pg/ml in the study group was greater than that in the control group(3.40±0.73)pg/ml,TNFα(5.22±0.47)ng/L in the study group was greater than that in the control group(3.87±1.33)ng/L,and EOS(2.58±0.34)%in the study group was greater than that in the control group(2.37±0.40)%.The IgA of the study group(0.23±0.03)g/L was less than that of the control group(0.72±0.25)g/L,and the IgG of the study group(1.33±0.31)g/L was less than that of the control group(5.28±0.74)g/L.Conclusions Indwelling nasal jejunal tube for nutritional support in patients with respiratory failure is helpful to improve nutritional status,enh
