表观弥散系数预测乳腺癌Ki-67表达水平的应用价值

摘　　要：目的:评估表观弥散系数(ADC)术前预测乳腺癌Ki-67指数的应用价值。方法:选取2023年6月—2024年8月于永州市中心医院经病理学证实为乳腺癌的151例患者的临床及影像学资料,根据Ki-67表达水平将患者分为Ki-67低表达组、Ki-67高表达组。测量对侧正常乳腺ADC值及病变最大ADC(ADC_(max))、平均ADC(ADC_(mean))、最小ADC(ADC_(min))值,并计算相对最大ADC(rADC_(max))、相对平均ADC(rADC_(mean))、相对最小ADC(rADC_(min))值。比较Ki-67低表达组与高表达组间ADC值的差异,并评估各ADC值对两组的鉴别效能。采用Spearman相关性分析评估乳腺癌ADC值与Ki-67之间的相关性。结果:Ki-67高表达组的ADC值均低于Ki-67低表达组,差异有统计学意义(P<0.05)。ADC_(max)、ADC_(min)、ADC_(mean)、rADC_(max)、rADC_(min)、rADC_(mean)值均能对Ki-67高表达组与Ki-67低表达组乳腺癌进行有效鉴别,其中ADC_(mean)对两组乳腺癌的鉴别效能最好。乳腺癌患者Ki-67水平与ADC_(max)、ADC_(min)、ADC_(mean)、rADC_(max)、rADC_(min)、rADC_(mean)值均呈负相关。结论:ADC值对乳腺癌的Ki-67表达指数具有一定的预测价值,可为患者治疗方案选择及预后风险评估提供参考。Objective ffffiTo evaluate the predictive value of apparent diffusion coefficient(ADC)in the expression of Ki-67 in breast cancer.Methods fiClinical and imaging data of 151 patients with confirmed breast cancer in Yongzhou Central Hospital from June 2023 to August 2024 were selected and divided into Ki-67 low expression group and Ki-67 high expression group according to Ki-67 expression level.The _(max)imum ADC value(ADC_(max)),average ADC value(ADC_(mean)),_(min)imum ADC value(ADC_(min))and contralateral normal breast tissue ADC were measured on ADC map,and the relative _(max)imum ADC value(rADC_(max)),relative average ADC value(rADC_(mean))and relative _(min)imum ADC value(rADC_(min)ff)were calculated.The difference of ADC value between ffiKi-67 low expression group and high expression group was compared.The discri_(min)ant efficacy of each ADC value was evaluated.Using Spearman correlation analysis assessed the correlation between ADC and Ki-67.Results The ADC value of Ki-67 high expression group was lower than that of Ki-67 low expression group(P<0.05).ADC_(max),ADC_(min),ADC_(mean),rADC_(max),rADC_(min) and rADC_(mean) ffvalues can effectively identify breast cancer between Ki-67 high expression group and Ki-67 low expression group,and ADC_(mean) ffffihad the best differential efficacy between the two groups of breast cancer.Ki-67 level of breast cancer patients were negatively correlated with ADC_(max),ADC_(min),ADC_(mean),rADC_(max),rADC_(min),and rADC_(mean).Conclusion ADC value can predict the expression level of Ki-67 in breast cancer,and provide a reference for the choice of treatment and the risk assessment of prognosis.
