摘  要:目的:分析超声剪切波弹性成像技术(SWE)在小肝癌诊断中的价值及相关参数。方法:选取2022年1月—2024年1月济宁市公共卫生医疗中心收治的87例疑似小肝癌患者,均行SWE、穿刺活检检查,以穿刺活检为金标准,分析SWE对小肝癌的诊断价值,比较小肝癌组与非小肝癌组的杨氏模量,包括标准差(SWE-SD)、最大值(SWE-max)、最小值(SWE-min)、平均值(SWE-mean),最后分析SWE对小肝癌的诊断价值。结果:87例疑似患者经金标准确诊42例小肝癌,其中SWE检出41例;SWE诊断小肝癌的灵敏度为95.24%、特异度为97.78%、准确率为96.55%、Kappa为0.931,与病理结果高度一致;小肝癌组SWE-SD、SWE-max、SWE-min、SWE-mean高于非小肝癌组,差异有统计学意义(P<0.05);受试者工作特征(ROC)曲线显示,SWE在小肝癌诊断价值的曲线下面积(AUC)均>0.70,其中联合诊断效能最高。结论:SWE在小肝癌诊断中价值高,可提高诊断灵敏度、准确率,指导早期治疗工作开展,改善患者整体预后,具有良好的临床应用价值。Objective To evaluate the diagnostic value of shear wave elastography(SWE)and its parameters in small hepatocellular carcinoma(HCC).Methods A total of 87 patients with suspected small HCC admitted to Jining Public Health Medical Center between January 2022 and January 2024 were enrolled.All patients underwent SWE and needle biopsy,with biopsy results serving as the gold standard.The diagnostic performance of SWE for small HCC was analyzed.Young's modulus parameters-including standard deviation(SWE-SD),maximum value(SWE-max),minimum value(SWE-min),and mean value(SWE-mean)-were compared between the small HCC group and non-HCC group.Results Among the 87 suspected cases,42 were pathologically fificonfirmed as small HCC,of which SWE detected 41.SWE demonstrated a sensitivity of 95.24%,specificity of 97.78%,accuracy fiof 96.55%,and a Kappa value of 0.931,indicating excellent agreement with pathological findings.The small HCC group exhibited fisignificantly higher SWE-SD,SWE-max,SWE-min,and SWE-mean values than the non-HCC group(P<0.05).Receiver operating characteristic(ROC)curve analysis revealed that all SWE parameters achieved an area under the curve(AUC)>0.70 for diagnosing ffismall HCC,with combined parameters showing the highest diagnostic efficacy.Conclusion fiSWE holds significant clinical value in diagnosing small HCC,improving sensitivity and accuracy,guiding early treatment decisions,and enhancing patient prognosis.
