双胎妊娠早产发生状况及危险因素分析

摘　　要：目的了解北京市东城区助产机构双胎妊娠产妇早产发生状况及趋势,并分析其影响因素。方法对2018年1月~2023年12月我助产机构分娩的双胎产妇967例,将分娩孕周为28~36+6周的产妇503人次作为早产组,分娩孕周大于等于37周464人次作为对照组。结果单因素:早产组的平均分娩年龄、高龄产妇占比低于对照组,产次均值高于对照组,(P<0.05);早产组中瘢痕子宫、胎膜早破、子痫前期、不良孕产史、孕前BMI>25kg/m^(2)、单绒毛膜性、双胎发育不均衡及经产妇比例均高于对照组,(P<0.05);logistic回归分析:胎膜早破、子痫前期、单绒毛膜性、孕前BMI>25kg/m^(2)、双胎发育不均衡、产次是导致双胎妊娠产妇发生早产的危险因素,(P<0.05)。结论多种因素能够导致双胎妊娠是双胎妊娠早产的危险因素,孕期应加强产检频次,尽早捕捉高危信号,减少双胎妊娠发生早产的几率。Objective To understand the incidence and trends of preterm birth among twin pregnancies in maternity institutions in Dongshen District,Beijing,and to analyze the influencing factors.Methods A total of 967 twin pregnancies delivered at our maternity institution from January 2018 to December 2023 were included.Among them,503 deliveries with gestational age between 28 and 36+6 weeks were classified as the preterm group,and 464 deliveries with gestational age≥37 weeks were the control group.Results Univariate analysis showed that the average age at delivery,the proportion of older mothers,and the mean number of previous pregnancies were lower in the preterm group than in the control group(P<0.05).The preterm group had a higher proportion of previous cesarean sections,early rupture of membranes,preeclampsia,adverse pregnancy history,pre-pregnancy BMI>25 kg/m^(2),monochorionic twins,unequal fetal growth,and multiparas compared to the control group(P<0.05).Logistic regression analysis indicated that early rupture of membranes,preeclampsia,monochorionic twins,pre-pregnancy BMI>25 kg/m^(2),unequal fetal growth,and previous pregnancies were risk factors for preterm birth in twin pregnancies(P<0.05).Conclusion Various factors are associated with the risk of preterm birth in twin pregnancies.It is recommended to increase the frequency of prenatal check-ups to detect high-risk signals early and reduce the incidence of preterm birth in twin pregnancies.
