摘  要:目的探究高原地区自体血回输在骨科大手术中应用效果。方法回顾性分析2018年1月~2024年2月在我院骨科手术患者79例,所有手术患者在术中均施行自体血回输,统计术中出血量和回输量。观察回输时和回输后7d内有无输血不良反应发生;比较术前和术后1d血常规和凝血常规指标。结果回输时及术后7d未发生输血不良反应;与术前相比,术后1d红细胞(RBC)、红细胞压积(Hct)、血红蛋白(HB)有所下降,(P<0.05);活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)和白细胞(WBC)有所上升,(P<0.05);血小板(PLT)和凝血酶原时间(PT)无明显变化。结论自体血回输在高原地区骨科大手术中应用,对患者术后凝血常规未造成明显影响,且无输血不良反应发生。Objective To explore the effect of autologous blood retransfusion in orthopedic surgery in plateau area.Methods A retrospective analysis was conducted on 79 patients who underwent surgery in the orthopedic department of our hospital from January 2018 to February 2024.All patients underwent autologous blood reinfusion during the operation,and the amount of bleeding and reinfusion was statistically recorded.The occurrence of adverse reactions to blood transfusion was observed during and within 7 days after reinfusion;the differences in hematological and coagulation parameters before and after surgery were compared.Results There were no adverse transfusion reactions at the time of reinfusion and 7 days after surgery.Compared to preoperative levels,there was a significant decrease in red blood cells(RBC),hematocrit(Hct),and hemoglobin(HB)at 1 day postoperative(P<0.05);there was a significant increase in activated partial thromboplastin time(APTT),fibrinogen(FIB),and white blood cells(WBC)(P<0.05);there was no significant change in platelets(PLT)and prothrombin time(PT).Conclusion Autologous blood reinfusion is applied in major orthopedic surgery at high altitudes,and it does not significantly affect the patient's coagulation parameters after surgery,and no adverse reactions to blood transfusion occur.
