从“阳化气,阴成形”探讨中医序贯辩治小儿肾病综合征

摘　　要：基于“阳化气,阴成形”理论,阐释小儿肾病综合征的核心病机为“阳化气”不足,肺、脾、肾三脏亏虚;“阴成形”太过,气血水瘀滞不通,并随激素用量的增减,“阳化气”与“阴成形”的太过或不及会序贯演变。治疗当扶阳化气,助肺、脾、肾三脏气化;消阴祛邪,利水、降浊、化瘀、行气以通其流,并随病机的序贯演变在激素初加时,扶阳化气、利水消阴;激素巩固维持期,养阴生津、清热活血;激素减停期,助阳益气、固肾健脾,进行中医序贯辨治。Based on the theory of‘yang transforming qi and yin constituting form',explaining the core pathogenesis of nephrotic syndrome in children is the deficiency of yang transforming qi'and the deficiency of the three viscera of lung,spleen and kidney.'Yin constituting form'is too much,qi,blood and water stasis is impassable,and with the increase or decrease of hormone dosage,‘yang transforming qi'and‘yin constituting form'too much or less will evolve in sequence.Treatment should reinforcing yang qi,help lung,spleen and kidney three zang-organs's qi transformation;dispeling yin pathogen,promoting urination,descending turbid,resolving stasis,and promoting qi to flow through its flow,and with the sequence of disease pathogenesis in the initial addition of hormones,reinforcing yang and transforming qi,promoting urination and dispeling yin.In hormone consolidation and maintenance period,nourishing yin and promoting fuid production,clearing heat and activating blood.In hormone reduction stop period,assisting yang and replenishing qi,solid kidney and invigorating spleen for TCM sequential differentiation and treatment.
