证候诊断视角下皮质醇与慢性阻塞性肺疾病基本证候相关性的思考

摘　　要：慢性阻塞性肺疾病(COPD)是呼吸系统常见病、多发病,其患病率高、致残率高、病死率高,且呈上升趋势,是全球公共卫生问题之一。中医药治疗COPD具有较好的优势,但目前中医证候诊断存在诊断标准形式各异、主观性强、难以量化、诊断缺乏与疾病临床表型(症状、体征、理化指标等)的有效整合等问题,影响了中医疗效的发挥。证候的准确辨识是中医辨证论治有效的前提,基本证候辨证有利于化繁为简,把握辨证核心,指导临床辨证,以建立客观、规范的证候诊断标准,提高中医的临床疗效。文章以皮质醇与COPD基本证候的相关性为基础,探讨证候诊断视角下理化指标对中医证候诊断客观化及精准辨证的价值。Chronic obstructive pulmonary disease(COPD)is a common and frequently occurring disease of the respiratory system,with high prevalence,high disability rate,high mortality rate,and an increasing trend.It is one of the global public health problems.Traditional Chinese medicine(TCM)has good advantages in the treatment of COPD,but there are some problems in TCM syndrome diagnosis,such as different forms of diagnostic criteria,strong subjectivity,difficulty in quantification,lack of effective integration of diagnosis and clinical phenotype of the disease(symptoms,signs,physical and chemical indicators,etc.),which affect the effectiveness of TCM.The accurate identification of syndrome differentiation is the premise of effective treatment of TCM syndrome differentiation.Basic syndrome differentiation is conducive to simplifying the complex,grasping the core of syndrome differentiation,guiding clinical syndrome differentiation,establishing objective and standardized criteria for syndrome differentiation,and improving the clinical efficacy of TCM.Based on the correlation between cortisol and basic syndrome of COPD,this paper explores the value of physical and chemical indicators in the objective and accurate syndrome differentiation of traditional Chinese medicine(TCM)from the perspective of syndrome diagnosis.
