中医药治疗慢性粒细胞白血病临床研究综述

摘　　要：慢性粒细胞白血病(CML)属于中医“慢髓毒病”范畴。根据古今的阐述,其病因病机主要归纳为正气不足、热毒炽盛、痰瘀互结3个方面,治疗上以扶助正气、清热解毒、化痰祛瘀为主,且在此基础上加用具有确切抗癌作用的中药。通过对中医药治疗CML临床研究的归纳总结发现,中医药在提高CML患者临床疗效、改善生活质量和降低不良反应等方面效果突出。文章在总结中医药治疗CML的临床研究现状基础上,从完善中医药临床研究方案、加强中医药逆转酪氨酸激酶抑制剂(TKIs)耐药和降低TKIs不良反应的机制研究、深入研究TKIs耐药和TKIs不良反应的中医证候本质,以及规范中医诊疗方案等方面提出建议,希望通过开展科学合理严谨的临床研究,结合中医特色化的基础实验研究,深入探索TKIs耐药和TKIs不良反应中医证候本质以及规范中医治疗方案来推动中医药治疗CML的现代化发展。Chronic myeloid leukemia(CML)belongs to'chronic marrow-toxic disease'in traditional Chinese medicine(TCM).According to ancient and modern explanations,its etiology and pathogenesis can be summarized into three aspects:lack of anti-pathogenic qi,fire toxin,and intertwined phlegm and blood stasis.The treatment also focuses on reinforcing healthy qi,clearing heat and removing toxins,and eliminating phlegm and transforming stasis.On this basis,some TCM with precise anticancer effects are added.Based on the summary of clinical studies on the treatment of CML with TCM,it is found that TCM has outstanding effects in improving clinical efficacy,improving quality of life,and reducing adverse reactions in patients with CML.On the basis of summarizing the current state of clinical research in the treatment of CML,recommendations are made in terms of refining clinical research protocols of TCM,strengthening the mechanistic studies of TCM on reversing resistance to tyrosine kinase inhibitors(TKIs)and reducing side reactions of TKIs,deeply investigating the essence of TCM syndromes on TKIs resistance and TKIs side reactions,and standardizing the protocols for the diagnosis and treatment of CML,in the hope of developing scientifically sound and rigorous clinical studies,combined with the characteristic basic experimental research of TCM to deeply explore the essence of TCM syndromes of TKIs resistance and TKIs side reactions as well as to standardize TCM treatment regimens to promote the modernization of TCM treatment of CML.
