摘 要:目的:观察大鼠慢性阻塞性肺疾病(COPD)造模前后肺俞、膀胱俞、大肠俞温度、微循环血流灌注量的变化,及电针改善COPD大鼠肺功能对穴位敏化的影响。方法:正常SD大鼠适应性饲养1周后,将30只大鼠随机分为正常组10只,模型组20只,造模成功后,又将20只COPD大鼠随机分为模型组和电针组各10只。毫针直刺尺泽穴5mm,参考电极在肺经线上尺泽穴旁开1mm,每日30min,疏密波,频率10/50Hz,强度1mA,以大鼠上肢轻度抖动为度,每周6次,连续干预4周。正常组、模型组除无针刺尺泽干预外,其他均与电针组相同。结果:与造模前比较,随着COPD大鼠模型的成功,肺俞、大肠俞以及膀胱俞穴位皮肤温度、微循环血流灌注量均显著升高(P<0.05,P<0.01);与正常组比较,模型组大鼠肺功能的肺弹性阻力出现上升(P<0.05),肺顺应性及肺每分通气量出现下降等肺脏功能下降情况(P<0.01),相应的肺俞、膀胱俞、大肠俞穴皮肤温度则出现升高等穴位敏化现象;电针COPD大鼠尺泽穴干预后,随着大鼠肺功能的恢复,其相应的肺俞、膀胱俞、大肠俞穴皮肤温度、微循环血流灌注量均出现下降(P<0.05,P<0.01),基本恢复到正常组水平。结论:COPD大鼠在肺俞、膀胱俞、大肠俞部位会出现皮肤温度、微循环血流灌注量升高等类似穴位敏化现象,与腑脏功能改变相关;电针尺泽穴改善COPD大鼠的肺功能,从而使得肺俞、膀胱俞、大肠俞部位升高的皮肤温度、微循环血流灌注量减低,穴位敏化随着肺脏功能的恢复而消失。Objective:To observe the change of skin temperature and microcirculatory blood perfusion units(MBPU)at the area of Feishu(BL 13),Pangguangshu(BL 28)and Dachangshu(BL 25)in the rats of chronic obstructive pulmonary disease(COPD),and to observe the effects of electroacupuncture(EA)in improving lung function in COPD rats on acupoint sensitization.Methods:After adaptive feeding for 1 week,rats were randomly divided into control group(n=10)and model group(n=20).Then the 20 COPD rats were randomly divided into model group(n=10)and EA group(n=10)after modeling successfully.EA group was treated with EA at the Chize(LU 5)point with filiform needle stabbing 5 mm,using the density wave(10/50 Hz,1 mA),30 min/time,6 times/week,which lasted for 4 weeks.The slight twitch of upper extremities was taken as the degree.Control group and model group were treated as same as EA group except EA therapy.Results:With the success of the COPD rats model,the skin temperature of Feishu(BL 13),Pangguangshu(BL 28)and Dachangshu(BL 25)increased significantly with statistically significant differences(P<0.01,P<0.05).Also,the MBPU at the area of Feishu(BL 13),Pangguangshu(BL 28)and Dachangshu(BL 25)increased significantly after modeling with statistically significant differences(P<0.01,P<0.05).Compared with the control group,the pulmonary elastic resistance of model group increased(P<0.05),lung compliance and the ventilatory capacity of the lung per minute decreased(P<0.01).Acupoint sensitization happened at the area of Feishu(BL 13),Pangguangshu(BL 28)and Dachangshu(BL 25)including the increasing of the skin temperature.After the EA at LU 5,the COPD rats'lung function recovered,the skin temperature of Feishu(BL 13),Pangguangshu(BL 28)and Dachangshu(BL 25)almost decreased to the level of control group(P<0.05,P<0.01).The effects of EA on lung function and MBPU of Feishu(BL 13),Pangguangshu(BL 28)and Dachangshu(BL 25)in COPD rats were consistent with the effects of skin temperature.Conclusion:COPD rats have similar acupoint sensitization phenomena
