肛瘘闭管汤对湿热下注型复杂性肛瘘术后创面的影响

摘　　要：目的:观察肛瘘闭管汤对湿热下注型复杂性肛瘘术后创面的影响。方法:将72例湿热下注型复杂性肛瘘患者随机分成对照组和治疗组,每组36例。两组患者均采用低位切开、高位挂线、支管之间以橡皮筋对口引流的手术方法。术后创面,对照组采用每晚碘伏换药1次为治疗方法。治疗组在对照组的基础上,加用口服肛瘘闭管汤治疗,疗程为14d。比较两组患者术后创面的愈合时间、创面愈合率、创面疼痛评分、创面分泌物评分、创面水肿评分及总体疗效。结果:治疗组创面愈合时间短于对照组(P<0.05);术后第14、28天治疗组创面愈合率均高于同期对照组(P<0.01,P<0.05);术后第7、14、28天治疗组创面疼痛评分和创面分泌物评分均低于同期对照组(P<0.01,P<0.05);术后第14天治疗组创面水肿评分低于对照组(P<0.01)。结论:肛瘘闭管汤可以加快湿热下注型复杂性肛瘘术后创面愈合速度,减轻术后疼痛,减少分泌物及减轻水肿,值得临床推广。Objective:To observe the effects of Ganglou Biguan Decoction on postoperative wound of complex anal fistula with damp-heat descending syndrome.Methods:A total of 72 complex anal fistula patients with damp-heat descending syndrome were randomly divided into control group and treatment group,each group had 36 cases.Both of the two groups were treated with surgical method of incision at low level,seton division at high level,and rubber band drainage between branches.For postoperative wounds,the control group was treated by applying iodine solution to the wound once every evening.The treatment group was treated additionally with oral medicine which was Ganglou Biguan Decoction on the basis of treatment in the control group,the course of oral medicine was 2 weeks.The healing time,healing rate,pain score,secretion score,edema score and overall effcacy of the wound were compared between the two groups.Results:The healing time of the wound in the treatment group was shorter than that in the control group after treatment(P<0.05).The healing rate was higher than that in the control group on the 14th and 28th days after surgery(P<0.01,P<0.05).The pain score and the secretion score of the wound in the treatment group were lower than those in the control group on the 7th,14th and 28th days after surgery(P<0.01,P<0.05).The edema score of the wound in the treatment group was lower than that in the control group on the 14th day after surgery(P<0.01).Conclusion:Ganglou Biguan Decoction can accelerate the healing of the wound,relieve postoperative pain,reduce secretions and reduce edema,which is worthy of clinical promotion.
