摘  要:目的 探讨以赫氏回(heschl's gyrus,HG)为种子点的全脑功能连接(FC)强度变化与老年性聋(PC)患者认知损害的关系。方法 选取PC患者(PC组)109例和正常对照者(HC组)63例,采用基于种子点的rs-f MRI FC算法,以双侧HG为种子点,分析全脑FC情况,找出两组FC强度差异显著脑区,并对所有受试者进行认知功能评估。比较两组认知功能指标(MOCA、AVLT、SDMT、Stroop、TMT-A)及听力相关指标(PTA、SRT),并对PC患者的全脑FC强度、认知功能指标及听力相关指标进行相关性分析。结果 以右侧HG为种子点,PC组有14个聚类FC降低(P_(FDR)<0.01,聚类大小≥20体素),包括左侧颞下回、双侧颞极、左侧颞中回、双侧岛叶、双侧颞上回、双侧壳核、双侧丘脑和双侧前扣带回。右侧HG和突显网络核心脑区(左侧岛叶、前扣带回、右侧丘脑)、左侧颞上极之间的FC强度与听觉词语学习测验(AVLT)得分呈正相关(r分别为0.3835、0.5811、0.3508、0.4259,P均<0.05);右侧HG和左侧岛叶间的FC强度与连线测试(TMT-A)所用时间呈负相关(r=-0.3885,P<0.01)。结论 PC患者右侧HG与突显网络核心脑区间的FC强度降低与认知损害相关。Objective To explore the relationship between changes in functional connectivity(FC)strength with Heschl’s gyrus(HG)as the seed point and cognitive impairment in presbycusis(PC).Methods 109 PC and 63 healthy controls(HC)were included.Resting-state functional magnetic resonance imaging(rs-fMRI)was used to obtain brain FC with bilateral HG as the seed point for both the PC and HC groups.Cognitive function assessments were conducted for all participants.The cognitive function indicators(MOCA,AVLT,SDMT,Stroop,TMT-A)and auditory-related indicators(PTA,SRT)were compared be-tween the two groups,and the correlation between the FC strength of the whole brain,cognitive function indicators,and hearing was analyzed in PC patients.Results 14 clusters of FC decreases were identified in the PC group(PFDR<0.01,cluster size>20 voxels),including the left inferior temporal gyrus,bilateral temporal poles,the left middle temporal gyrus,bilateral insula,bi-lateral superior temporal gyrus,bilateral putamen,bilateral thalamus,and bilateral anterior cingulate and paracingulate gyri.The FC strength between the right HG and the core nodes of the salience network(left insula,anterior cingulate and paracingu-late gyri,right thalamus)and left superior temporal pole were positively correlated with the scores of the AVLT(r=0.3835,0.5811,0.3508,0.4259,P<0.05).The FC strength between the right HG and the left insula was negatively correlated with the time used in TMT-A(r=-0.3885,P<0.01).Conclusion The alterations in FC strength between the right HG and core nodes of SN in PC are associated with cognitive impairment.
