产前MRI及超声诊断胎儿后颅窝囊性畸形的对照分析

摘　　要：目的 探讨总结胎儿后颅窝囊性畸形的产前磁共振(MRI)表现并与超声检查(US)结果对照,评价MRI检查对胎儿小脑蚓部发育异常的产前诊断价值。方法 选取产前US检查提示胎儿后颅窝囊性畸形孕妇30例,3日内行MRI检查,对出生后的患儿行胎儿MRI检查,或对引产胎儿行尸检,并与产前US、MRI结果对照,评价US和MRI检查的诊断准确率。结果 本组随访病例证实的13例胎儿Dandy-Walker畸形(DWM),1例Blake囊,7例大枕大池及9例枕大池蛛网膜囊肿。胎儿MRI除1例误诊,对13例DWM及合并畸形全部检出,US漏、误诊11处,15例出生后随访枕大池蛛网膜囊肿和大枕大池无变化。结论 MRI有助于明确诊断胎儿小脑蚓部发育异常及伴发畸形,可作为胎儿US有利的补充及验证手段,进一步完善产前诊断。Objective To summarize the features of ultrasound(US)and magnetic resonance imaging(MRI)for the fetal posterior fossa cystic abnormalities and to assess the value of US and MRI in the prenatal diagnosis of fetal cerebellar vermis dys-plasia.Methods 30 cases of pregnant women with posterior fossa cystic abnormalities were studied with a 1.5 T superconduc-tive MRI unit within 3 days after ultrasound examination.Diagnostic accuracy of MRI and US was evaluated by comparing with prenatal diagnosis of MRI,US and postnatal diagnosis of MRI or postmortem examination.Results This group of fetal MRI con-firmed the 13 cases of fetal Dandy-Walker malformation by follow-up,1 case of Blake capsule,7 cases of big pillow large pool,and 9 cases of arachnoid cysts in pillow pool.Except one case of diagnosis,of 13 cases of DWM and merge deformity subjected to MRI detection,all were examined by MRI,11 deformities were misdiagnosed by US.15 cases of fetus had no change in arach-noid cyst and cistern after birth.Conclusion MRI is helpful to diagnosis of fetal cerebellar vermis dysplasia and secondary de-formity,which is a beneficial supplement and verification method to fetus US,further perfecting prenatal US diagnosis.
