腔内三维超声成像后处理技术对早孕期胎儿神经节隆起的应用价值

摘  要:目的 探讨应用腔内三维容积探头观察早孕期正常胎儿颅内神经节隆起(ganglion eminence,GE),探讨其评估早孕期GE生长发育规律的可行性。方法 选取进行早孕期筛查的孕妇192例,应用腔内三维容积探头后处理技术获取胎儿神经节隆起的面积、体积,包括经胎儿丘脑横断面、丘脑旁矢状面及丘脑前冠状面的单侧GE面积和单侧GE体积,分析胎儿单侧GE的面积及体积与头臀长(crown-rump length,CRL)及头围间的相关性。结果 组间及组内的重复性具有良好的一致性,组间ICC范围为0.984~0.993,组内ICC范围为0.990~0.997。选取的三个切面单侧GE面积均与CRL及头围呈明显正相关,r~2值范围为0.678~0.844。单侧GE体积与CRL及头围亦呈明显正相关,r~2值范围为0.754~0.894。结论 本文应用腔内三维超声后处理技术显示GE的全貌,并建立11~14周的胎儿单侧GE的正常参考值范围,为评估早孕期GE发育、早期发现GE发育异常提供诊断依据。Objective To investigate the feasibility of intracranial ganglion eminence(GE)in normal fetuses in early preg-nancy by three-dimensional volumetric probe,to quantify its size,as well as to evaluate the growth and development of GE in early pregnancy.Methods 192 cases of pregnant women undergoing early pregnancy screening were selected.The area and vol-ume of the fetal ganglionic eminence were acquired using post-processing techniques with a three-dimensional volume probe in the application cavity,including the unilateral GE area of the transverse section,para-thalamic sagittal plane and anterior coro-nal plane of the fetal thalamus and unilateral GE volume.The correlation between the area and volume of unilateral fetal GE and the crown-rump length and the head circumference of the fetus was analyzed.Results The reproducibility between and within groups was consistent,with ICC ranging from 0.984 to 0.993 and ICC ranging from 0.990 to 0.997 between groups.The unilateral GE area of fetal thalamus cross-section,paramidcerebral sagittal plane and anterior thalamic coronal plane was significantly posi-tively correlated with crown-rump length and head circumference,and the r²value ranged from 0.678 to 0.844.Unilateral GE vol-ume was significantly positively correlated with CRL and head circumference(r2=0.754-0.894).Conclusion This study ap-plied intracavitary three-dimensional ultrasound post-processing technology to display the full picture of GE,and established the normal reference range for unilateral GE in fetuses aged from 11 to 14 weeks,offering a diagnostic basis for assessing the devel-opment of GE in early pregnancy and early detection of abnormalities in ganglion eminence development.
