摘 要:目的 探讨在受硬化伪影影响的颈根部CT图像中,比较辐射剂量相似、固定扫描条件下常规CT与能谱CT的图像质量。方法 选取使用Discovery 750 CT获得95例常规CT扫描(A组)及95例能谱颈椎间盘扫描患者(B组)影像学资料,190例患者一般资料比较差异无统计学意义(P>0.05)。感兴趣区选择C6/7椎间盘和同层面硬膜囊,采集CT值、噪声值,计算对比噪声比(CNR)和硬化伪影指数。对比组间上述参数和图像主观评分差异。另逐对比较A组、B组图像的CNR值,有70对样本能谱扫描图像的CNR值比常规图像高,其内70例常规扫描患者分组为C组;剩余25对样本能谱扫描图像CNR比常规图像低,其内25例常规扫描患者分组为D组,对比C组、D组图像参数差异。结果 A组、B组比较,B组图像CNR值及评分高,脊髓噪声、BHAI低,差异均有统计学意义(P <0.05),能谱扫描提升了图像质量。C组、D组比较,C组图像CNR值及主观评分低,脊髓噪声及BHAI高,差异均有统计学意义(P <0.05);C组脊髓噪声的95%CI为11.06~13.62 HU,在此噪声范围能谱较常规扫描有提高CNR的能力;D组脊髓噪声的95%CI为8.01~10.69HU,在此噪声范围能谱较常规扫描失去提升CNR的能力。结论 相同辐射剂量下,较常规CT扫描,固定扫描条件的能谱CT可以在部分样本中减低噪声,提高组织对比度,改善硬化伪影对颈根部图像质量的影响。Objective In the neck root computed tomography(CT)images affected by hardening artifacts,comparison of im-age quality between routine and energy spectrum under similar radiation dose and the fixed scanning conditions was discussed.Methods Discovery 750 CT was used to obtain 95 cases of conventional cervical disc scanning and 95 cases of energy spectrum scanning,and they were divided into group A(routine scanning)and group B(energy spectrum scanning).There was no signifi-cant difference in the general data of 95 cases.C6/7 disc and dural sac were selected as areas of interest.CT and noise values were collected,and comparing noise ratio(CNR)and beam-hardening artifacts index(BHAI)were calculated.The differences of the above parameters and image subjective scores were compared between groups.The CNR values of group A and group B were compared one by one.There were 70 pairs of samples with higher CNR in energy spectrum scanning images than in conven-tional images,and 70 cases with conventional scanning were classified as group C.The CNR of the remaining 25 pairs of samples was lower than that of the conventional images,and the 25 cases of the conventional scan were grouped into D.The difference of image parameters between group C and D was compared.Results Compared with group A and B,the CNR value and score of group B were higher,and the spinal cord noise and BHAI were lower,the differences were statistically significant(P<0.05),and the image quality was improved by energy spectrum scanning.Compared with group A and B,the CNR value and score of group B were higher,and the spinal cord noise and BHAI were lower,the differences were statistically significant(P<0.05),and the image quality was improved by energy spectrum scanning.Compared with group C and D,CNR and subjective score of group C were lower,spinal cord noise and BHAI were higher,and the differences were statistically significant(P<0.05).The 95%confidence interval of spinal noise in group C was 11.06~13.62 Hu,and the CNR could be improved by energy spectrum sc
