新型多功能输尿管硬镜内切开联合球囊扩张治疗输尿管上段超短段针孔样狭窄1例报告并文献复习

摘　　要：目的:探讨输尿管狭窄的腔内治疗方式以及新型多功能输尿管硬镜治疗输尿管狭窄的安全性与有效性。方法:回顾性分析1例采用新型多功能输尿管硬镜治疗的输尿管上段超短段针孔样狭窄患者的临床资料,结合相关文献对其临床诊治进行讨论。结果:该患者手术过程顺利,手术时间52 min,术中出血量5 mL,术后住院时间3 d。术后随访6个月,患者肾积水完全缓解,肾功能维持稳定。结论:针对狭窄程度严重的病例,可以考虑采取输尿管镜内切开术联合球囊扩张术,给患者提供一次避免重建手术的机会。对于输尿管狭窄的腔内手术治疗,新型多功能输尿管硬镜提供了新的解决方案,采用该镜进行上尿路疾病的诊疗操作,手术过程简便,治疗效果确切,无严重并发症,在未来需大样本临床队列进一步证实其安全性与有效性。Objective:To study the endourological treatment of ureteral strictures and discuss the safety and efficacy of a new multifunctional rigid ureteroscope for the treatment of ureteral strictures.Methods:Combined with reviewing the relevant literature,we retrospectively analyzed the clinical data of a patient with needle-like stricture in the upper segment of the ureter treated with the new multifunctional rigid ureteroscope.Results:The surgery was performed successfully,with a duration of 52 minutes and an estimated intraoperative blood loss of 5 mL.The postoperative hospital stay was 3 days.Follow-up data at 6 months showed complete resolution of renal hydronephrosis,with stable renal function.Conclusion:In cases with severe stricture,ureteroscopic endoureterotomy combined with balloon dilatation can be considered to give patients a chance to avoid reconstructive surgery.For endoluminal surgical treatment of ureteral strictures,the new multifunctional rigid ureteroscopy provides a new solution.The use of this ureteroscope for diagnosis and treatment of upper urinary tract diseases is simple,with precise therapeutic effects and no serious complications,but its safety and effectiveness will need to be further confirmed by a large clinical cohort in the future.
