摘 要:目的基于横断面及孟德尔随机化(MR)研究探讨血脂异常与高血压发病风险的关系。方法回顾性收集2011—2018年美国国家卫生与营养检查调查18岁以上患者数据进行横断面研究,包括高血压组2213例和非高血压组3790例;比较两组患者临床资料,采用多因素加权logistic回归分析高血压发病的影响因素。收集公开发表的国际流行病学联盟开放型全基因组关联研究相关数据,采用多变量MR分析中的逆方差加权法分析总胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)及甘油三酯(TG)与高血压发病风险的因果关系。结果高血压组与非高血压组患者血清HDL、TG水平比较,差异均有统计学意义(均P<0.01);两组患者血清LDL、TC水平比较,差异均无统计学意义(均P>0.05)。校正潜在混杂因素后发现,TG(OR=1.17,95%CI:1.03~1.32,P=0.017)是高血压发病的独立影响因素。MR分析结果显示,TG(OR=1.05,95%CI:1.02~1.08,P=0.001)与高血压发病风险存在因果关系。结论高血压患者多合并血脂异常,高TG水平对高血压发病风险影响较大。Objective To explore the relationship between dyslipidemia and the risk of developing hypertension by conducting a cross-sectional and Mendelian randomization(MR)-based study.Methods Data on patients aged 18 years or older from the 2011-2018 American National Health and Nutrition Examination Survey were collected for the cross-sectional study,including 2213 cases in the hypertensive group and 3790 cases in the non-hypertensive group.Clinical data were compared between the two groups,and the factors influencing the development of hypertension were analyzed using multivariate weighted logistic regression.Relevant data from open Genome-Wide Association Study of the International Union for Epidemiology published in the public domain were collected,and the causal relationships of total cholesterol(TC),high-density lipoprotein(HDL),low-density lipoprotein(LDL),and triglyceride(TG)with the risk of developing hypertension were analyzed by using inverse variance weighting method in multivariate MR analysis.Results Comparison of serum HDL and TG levels between patients in the hypertensive group and the non-hypertensive group showed statistically significant differences(both P<0.01),while comparison of serum LDL and TC levels between the two groups showed no statistically significant differences(both P>0.05).After correcting for potential confounders,TG(OR=1.17,95%CI:1.03-1.32,P=0.017)was found to be an independent factor influencing the development of hypertension.The results of MR analysis showed that there was a causal relationship between TG(OR=1.05,95%CI:1.02-1.08,P=0.001)and the risk of developing hypertension.Conclusion Hypertensive patients have a high possibility of dyslipidemia,and high TG level has a relatively greater impact on the risk of developing hypertension.
