摘　　要：目的探讨血液透析患者并发高血压的影响因素。方法回顾性选择2022年1月至2023年6月余姚市人民医院就诊的血液透析并发高血压患者90例作为观察组,另选取同时期本院血液透析未并发高血压患者90例作为对照组,比较两组患者的一般资料和实验室检查指标,采用多因素logistic回归分析血液透析患者并发高血压的影响因素,采用ROC曲线分析血液透析患者并发高血压影响因素的预测效能。结果观察组促红细胞生成素用量、透析间期体重增长率、透析前血钠、镁高于对照组,而尿素清除指数(Kt/V)值低于对照组,差异均有统计学意义(均P<0.05)。多因素logistic回归分析显示,透析间期体重增长率和Kt/V值是血液透析患者并发高血压的独立影响因素(均P<0.05)。ROC曲线分析显示,透析间期体重增长率联合Kt/V预测高血压的AUC、灵敏度和特异度分别为0.882、0.766和0.857。结论透析间期体重增长率和Kt/V与高血压的发生相关。Objective To analyze the influencing factors of hypertension in hemodialysis patients.Methods A total of 90 hemodialysis patients with hypertension who received treatment at Yuyao People's Hospital from January 2022 to June 2023 were selected as an observation group,and 90 hemodialysis patients without hypertension were selected as a control group.Clinical data and laboratory measures were compared between the two groups.Logistic regression was used to analyze the factors influencing hypertension in hemodialysis patients.The ROC curve was used to analyze the predictive power of influencing factors for hypertension.Results Erythropoietin(EPO)dosage,Interdialysis weight gain,pre-dialysis serum sodium and magnesium were significantly higher,and urea clearance index(Kt/V)was significantly lower in the observation group than in the control group(all P<0.05).Multivariate logistic regression analysis showed that interdialysis weight gain and Kt/V were independent influencing factors of hypertension in hemodialysis patients(both P<0.05).ROC curve analysis showed that the AUC,sensitivity and specificity of predicting hypertension by combining interdialysis weight gain with Kt/V were 0.882,0.766,and 0.857,respectively.Conclusion Interdialysis weight gain and Kt/V are associated with hypertension development.
